病人 has MCA stroke, 同时还有junctional arrhythmia, HR42, low urine output,
dehydration.
在tibia上打一个孔，接上infusion fluids, and meds.
works well for adult patients.
顿时想到UW（CK？）有一题考的就是IO。只不过是儿童病人。 现在adult patients 也
用。 让我感觉到UW出的题目还是比较紧扣临床实践，考查实际中重要的知识。
我的感受UW不光考试有用，对临床也很有帮助。

I can't be agree more with what THR wrote "Nurses in med/surg can pretend

在CARDIAC FLOOR, 我们有standing order for potassium and magnesium
replacement. 保持 K> 4/4.5, MAG> 2。当然，如果病人有renal insufficiency , MD
会自己写医嘱。 低钾低镁有可能引发各种arrhythmias, 如果病人run NON-SUSTAIN
VT, 先看清楚是不是artifact, 量血压，问问病人当时在干什么，再数EKG STRIP有多
少BEATS，打电话给医生前，最好查查病人最近的POTASSIUM和MAGNESIUM是多少，因为
医生第一句话就会问。
还有hyperkalemia 也很常见。处理的方法 IVD50 PLUS 10U REGULAR INSULIN,
KAYEXALATE 30G PO, IV ca gluconate 1amp if EKG changes, 最后可能上DIALYSIS。 |

病人申诉“I CANN'T BREATH”第一步，还是要ASSESSMENT，VS 包括RR，SAT O2，有没
有nostril flare/using accessory muscle ，结合病史找原因，Pulmonary: PNA,
Pneumothorax, PE, Aspiration, bronchspasm, airway obstruction.
Cardiac: MI, CHF, arrhythmia, , 还有acidosis, sepsis, anemia, anxiety. 有没
有给新药？（allergic reaction), 肺音（fluid overload/asthma?). 先给氧气，酌情
处理，从simple face mask 到non- rebreather, 再打电话给医生，通常，会order
CXR, EKG, ABG.lasix, breathing treatment(beta agonist).这个时候RT 就是好伙伴
了，要找CHARGE帮忙，必要的时候CALL RAPID RESPOND TEAM, 最好ABG PO2〉 |

感觉：考下来感觉难度一般。个别问题觉得没有正确答案，个别问题完全不知道怎么答

arrhythmia under stress. My Total DL dropped down to 50-70s shortly and
HDL was higher than my LDL. My
NP was confused by the results and asked me to stop taking them. I believe
fish oils do exude some magic powers here. I used to have hypotension as
well, if that matters.

看很多人问这个考的多不多，那个多不多，每个人的考试侧重点会完全不一样，fioro&
#8232;计算和药物反应不多，不代表你不会被考到。我的计算就很多，也不代表你错的
多，如
果看bulletin,计算的比例就很高，而且好几道TPN的。主要作为一个参
考，而不是复习
的指南。我还被考到几题一样的，我确定我答的是对的，所以
也不要freak out。
我的最后也有profile的题目。
HIV也被考到了
brand 和generic，所以什么都可能考，不过NAPLEX就是一个minimum
competency的考试，大家肯定没问题的。

刚刚查了naplex分数，过了，分数居然还不错。 考完觉得要fail了。我的考题是rare
diseases，rare drug names and dosage forms 占据的了整个考试。其他的常
见的东
西也都有，但是都是蜻蜓点水的考一下，一两题的样子。计算也不多。

所以医生认为不用给rate control的药再降心跳了
用amio可能有原因的，比如心跳已经控制在50，但还出现心律不齐，特别是
ventricular arrhythmia, 就上rhythm control了
不知道完整的检查和诊断很难做出绝对的判断
就药而言，amiodarone能避免绝对应该避免用，副作用太强 |

antivert which is anticholinergic. Instead, I would use benzo for nausea
only, which also helps the anxiety.
Call me crazy but I think it is reasonable to put her back on amiodarone.
1. Amiodarone has nothing to do with her nausea.
2. Hypothyroidism can be corrected with synthroid.
3. Asymptomatic (cardiac wise)when on amiodarone.
4. A good agent for ventricular arrhythmia and A-fib.

上周的 journal club, 是一个高年fellow 讲NEJM Jul.8 2010 的adverse events
associated with testosterone administration, 也就是令老年人比较沮丧的TOM
trial.
Trial的本意，是想证实 65 岁以上、睾丸酮水平轻、中度降低的老男人，接受较大剂
量的睾丸酮治疗之后，可以增加 muscle strength and mass. Trial 进行下去，这个
预期结论是证实了，却同时发现，在睾丸酮治疗组里，cardiovascular event （MI,
ACS, arrhythmia, CHF, sudden death）的发生率是22%，而对照组仅有5%！以前的研
究仅有零星的报道说睾丸酮可能增加cardiovascular event，但没有形成气候，远不象
这个data 那么瘆人。于是大惊之下，就给这个trial 在六个月时候做了中期引产。

strong history of ischemic heart disease and had a CABG 2 years ago. How is
her LV function? If she has a poor LV function (say EF less than 40%) she is
at high risk of having ventricular arrhythmia which can present with
syncope or even sudden cardiac death. However, if posterior circulation
