"kentucky",s3 is -chy ; s4,s1,s2 is "tennesse",s4 is "tenn--".
to differentiate murmur, first identify it's systolic murmur or diastolic
murmur. When you ausculate heart, also press carotid pulse. If murmur after
pulsation, it is systolic. if murmur before pulsation, it is diastolic. then
you need identify it is aortic murmur or mitral valve murmur. Move your
stethscope, find the area where you heard murmur sound loudes |

家分享一下吧。
2010年6月中旬，既蠢又懒的我呕心沥血的终于熬到毕业。和别人share Kaplan, 把考
试日子定在了8月6日。 毕业的兴奋冲晕了头脑，做了件相当蠢的事：把所有书和资料
都卖了，认为1个多月应该把重点放在做题而不是看书。可悲的是每天做不到50题，成
绩是40%-50%。居然毫无危机感。（自大啊，太高估自己了。导致最后悲剧的发生。）
8月6日，12：30pm开考。边做心就边沉，似乎都不难，但没多少是确定的。 于是安慰
自己慢慢做，每个选项都仔仔细细的想完又想（又一致命错）。结果是越想越不确定。
做到75题没出现期望中的黑屏，一阵失望。很快到了第一个休息时间，当时想100题就
完了吧。 可是没有。 110，120，130。。。真希望下一题就黑屏。可是没有！ 等来的
是第二个休息。开始烦躁，怎么那么没完没了？！ 160，170，180。。。如坐针垫。
做到200题， 我已经崩溃了。眼睛越来越模糊，肚子越来越饿，心越来越凉，凉得身子
开始发抖， 大腿坐得早就发麻了。

1. Did you make the diagnosis on CXR or CT of the chest?
2. Did you give the TPA?
BTW, Do not use AAA for ascending aortic aneurysm. I know what you mean, but not the others. |

的偏瘫。 一般stroke是没有pain的， 当然有些posterior fossa的stroke可能会引起
headache。
ascending aortic dissection可以引起ischemic neuropathy。 我想病人的症状是
brachial plexus ischemia。 如果没有gut给TPA， heparin是必须的。

发帖数: 367 | 14 Carotid stenosis and renal artery stenosis at 60-70% is not critical. I have
seen a lot of patients follow even more severe stenosis.
Your father eventually needs diagnostic angiogram with aortic and bilateral
lower extremities run-off to evaluate for occlusion. Or he should at least
get arterial dopplers to check the waveforms and ABI (ankle brachial index)
first. |

（响应征文号召，翻出在纽约做心脏科Fellow时的一篇文章以表心意。）
周一晚上值班，周二休息。周一下午离开X医院心内科时，心脏监护病房（CCU）只有2
个病人，并且其中的一个将会转去普通病房。普通监护病人（Telemetry）还有6-7个。
所以，周三（圣诞夜）早上上班前，我对家人说，今天中午吃完午饭就可以回家了。圣
诞夜嘛，应该是很平静的。
07：30
早上7：30到了医院。刚拿到病人流程单，我的住院医生就对我抱怨了:“G医生，昨天
我们忙的不得了。6个新病人，我们查房就查到了下午4点”。可以理解，我们的
Attending C医生工作效率不是很高，查房时，不时会打打电话或和路过的同事聊聊天
。但是，查到下午4点也是有点太出格了吧。
我看了看流程单。CCU一个老面孔，今天已经预约了手术室做气管切开术，另外多了5个
新面孔。一个是心电监护组的病人，由于发现6秒的心脏停搏周一晚转到CCU，周二放置
了永久心脏起搏器，今天完成肾脏透析后可以出院；第二个是29岁的镰刀贫血智障患者
，昨天因急性心梗收入院，今天做换血治疗；第三个是70多岁的妇女因为急性心衰，昨

新手勿怪，不过估计是很多人想知道的吧。
您推荐的那本书我越看越糊涂，每个阶段列的例子似乎都穷举了所有可能的走势，看不
出来共同点。
能问下您看的是哪种图么？最基本的比如google financial上面candlestick + volume
就可以么？还是需要结合level II data,甚至historical level II data（这个是需要
买的吧）？本版一篇旧文里提到要看成交次数／平均成交量，这个在哪里可以看？
您曾经说做短线就是体力活，要盯盘，可是在评论大家的股票的时候，您并没有盯盘吧
？这样的信息量是否要少？
还有请问您screen stocks有什么软件推荐么？
