talk to manager first, give him 2 letters (one for him and one for HR, same
content). do not seal the envelopes, it's kind of awkward if he has to tear
it open in front of you.
you may ask why two same letters? because most likely HR will keep a copy in
their files, and you can't be sure your manager will 100% give "his" copy
to them. also, it's more polite to give him a separate one.
the most important thing in the letter is to clearly specify your departing
date, so HR can coordi

and then write email to manager and CC HR formally.
