My son has food allergy. He started preschool last year.
The beginning of this year, the pre-school asked a allergy form to be signed
by pediatrician.
I faxed form in and called to make sure they got it. I was told they will
called when the form is done.
I didn't hear from them for a month. I went there, but they cannot find the
signed form.
I called them 2 month later. The form is still not signed.
I faxed it in again today. When I call, they told me they are n |

mm们都怎么对付这些学校的表格，体检表没问题，一年一次。
allergy表那？我们年底体检的时候根本不知道有这么一个表。错过体检时间doctor就
不愿意给填了，怎么办。
明年我打算体检前找学校要表，提前填。
