As of August 1, 2003, an applicant seeking a change of status to or from F2
or M2 dependent classification must be issued a Dependent Copy of SEVIS Form
I-20, Certificate of Eligibility for Nonimmigrant Student Status. Please
submit the properly issued original Dependent Copy of the SEVIS I-20 for
each person with a dependent status included in your application. The form
must contain original signatures of both the student and designated school
