1) Usually yes, but it worth the effort to call the insurance and make sure
all your questions confirmed before delivery

3) Exam - yes. Cost can be billed to father's insurance plan - it goes
retrospectively in such case.
Again - it is just the policies that I known. You'd better read your policy
and double-check or triple check with the insurance company.
