Actually I am considering 2 choices, another is paying $140 more per month before tax in insurance premium, but 100% covered.
Do you know which one is better?

suppose.
OOP for that 90% one is $1500, deductible is $250.
But the monthly cost for 100% is $140 higher before tax.
So if the rough estimate of the total cost is $5000+, I probably will go
with the 100% coverage.

for answers to this.
Total charges from hospital can vary a lot from each other.
From what you wrote, covering 90% is 90% of amount of charge approved by
insurance company after deductible.
For example, assuming you have 1000 deductible. Hospital charges you 20K for
the whole thing, Insurance company will negotiate with them and the total
charge will get paid now becomes 12K. You will pay 1000 (ded.) + 10% * 11000
= $210 | f**g
