it's from metabank, OK.
old cards start with non 42xx won't be able to PIN--even it is from metabank.

at the cost of $2.50. After receiving the prepaid card, call customer
service at 1-866-923-0052 to cancel the card, and you’ll get a mailed check
with the card balance minus $2.50.
