胰岛素，这都什么年代了，中国都不这样了。

reimbursement, 政府想cut就cut,这以后根本负担不起的时候不就要大砍价吗？医生收
入这都多少年没涨了？加上通胀的话实际收入就是在下降。

吧。下载一个kaiser的app，看看里面有什么不就完了。您不是版主吗？能与时俱进一

发帖数: 1596 | 16 Kaiser 的医生愿意回email，是因为这样可以减少office visit啊，因为他们都是工资
制，费用都是预收的，所以不需要增加visit来创收，自然医生愿意干。他们很多
teleconsult，之所以专科医生愿意干，就是因为这样减少他们工作量。
不过虽然都是Kaiser，各地的还是很不一样，我原来一个病人是东岸Kaiser的，我们想
让他转到这边的Kaiser人家还不收。

够做到让医生和护士们能够高兴的提供能够给予的服务，皆大欢喜，那是理想的状态了
。
医疗改革是很大的一个工程。有时政客的出发点，是不错的，为吸引人，往往理想化，
可能都是一厢情愿。 能不能实现，合不合历史的潮流，如何能实现，都是摸着石头过
河。 就像 Obamacare，长期的影响是什么，身在河流之中，谁也说不准。
医院一个研究伦理的老人说，几十年前Social Security，Medicare等等，哪个政策出
来都是争论得很厉害，不比现在的程度少。 他们的后果如何，需要以开放的心态，等
待时间的答案。 可能，资本主义不可抗拒的走向社会主义，中国不可抗拒的回归资本
的阶段，都是历史和时间的规律。
就像日本明治维新，各派打得死去活来，谁也不是没有自己的道理。最后日本还是不可
抗拒的走向开放，幕府的封建制度走向没落。
Lumix的思维方式，理想程度，和一线临床医生不一样。甚至有一些像经常给我们宣教
的APN/RN管理者。就像住院医生一旦当了住院总，自然,讲话思维方式和他们几个月以
前非常不一样，高大上，lol. 象小丸子那样就是一般医生的想法，所以更象医生。

有smart phone吗？看的非英语人群？不要讲西班牙语，波兰语，意大利语呢？
average cost 是多少？有没有想过有人没月5刀的药钱都不负。
uimited.visit 没问题，但病人动不了没人送咋办？
还有同一天看专科，根本没人refer 你看谁？即使看到了，看的医生5分钟打发走了，
不给你解决问题你有啥办法？
这道理就像快餐和高级餐馆类似。不是所有人只吃快餐就满足的

other private practice doctors and doctors going to private practice. They
will face Walmart like competition and these type of competition is unfair.
As a physician, kaiser type practice will eventually affect all physicians:
low salary, high work intensity and millions of rules.

No. 8 may be the trend right now
No.9 has been done for many years in big centers.
No.10 has been institutionalized in many centers but the data won't be in
the cell phones, a practice against HIPPA.

No.12, possible but not necessary and does not make sense for a good
practice.
No.13, too many legal issues
