发帖数: 124 | 3 Traumatic macular hole can spontaneously resolve, but relatively rare. The
usual recomended watch period is 2 to 6 weeks post trauma. Based on your
case: a pediatric patient, vision is severely reduced, even show early sign
of retinal detachment; I would say surgery earlier rather than later to
prevent further detachment. It would be vitrectomy--followed by gas bubble
replacement.
And again, we can't stress enough about proper eye protection during sports.
This is one of the leading cause for monocular blindness in young people.
Every pediatric patient I see, I always ask about their sports activity and
recommend protective eyewear accordingly. It's always better to prevent

status of macula. Since there is macula impairment, earlier surgical
intervention is indicated.

发帖数: 124 | 6 I am not saying that. Because most of the traumatic retinal hole, tear or
detachment actually happens at the peripheral retina,we want to treat it
ASAP due to the potential thread to involve macula. At periphery, we don't
need to worry about central vision,so LASER can be used to re-seal the break
. And for large detachment,further vitrectomy and gas treatment might be
considered. It's the macula that's so delicate and can not be LASERed.
Because the possibility of spontaneous resolution, we can watch for a bit
more time.
Anyway, I am not an expert on this. Just know some basic principles. Hope
the kid can recover some of his vision in this eye.
