发帖数: 404 | 2 Does he have known metastasis? If it is locally advanced tumor which is
unresectable, the strategy is systemic chemotherapy +/- radiology with the
goal of shrink the tumor and make it resectable. If it is metastatic disease
, the main treatment is systemic chemotherapy for palliation. If he has
very good performance status, use FOLFIRINOX; with OK performance status,
use gemicitabine based chemo regimens. If he has poor performance status,
recommend t focus on comfort care and no chemotherapy. There is no magic
therapy in the US. In general, advanced pancreatic cancer has very poor
prognosis. |

I do not know much about the details yet. Not quite sure if I can still
visit this website in China. I am planning flying back to China next week
anyway.
My question is whether I should bring him back to seek better treatment (Is
US better than China?) and will he still be able to make it? Which hospital
/doctor we should go if he comes here? Or all the doctors follow the same
procedure as you described above and no difference? How much time left for
him? What I can do to save him or make him happier?

发帖数: 404 | 4 He will need to have CT of chest, abdomen, and pelvis to stage the disease
if not done already. Like I said, advanced pancreatic cancer is very poor
prognosis. Studies show people with metastatic disease in average live about
11 months with the best standard treatment (FOLFORINOX); without treatment,
the mean survival is about 6 months. Even people with early and resectable
disease, only 20% of them can be cured. This is the sad and realistic fact.
I doubt that bring him here is a better option, since that will delay his
treatment, if he currently is fit for chemotherapy. Physicians in China have
access the standard NCCN guildlines in the US. There is Chinese version of
NCCN guildlines online, though I have no idea in reality if they follow the
guildlines here. The best cancer centers in the US is MD anderson cancer
center in Houston, and Memorial Sloan-Ketting cancer center in New York, if
you want the names.

10cm至少是T3了。手术机会极微。 但如果是neuroendocrine carcinoma，那是完全不
一样了。 可以针穿活检一下。
如果是adenocarcinoma，若想争取手术机会，一定首先要放疗五周，附以一种化疗药。
再用PET CT 看效果。
感觉美国治疗胰腺癌会比国内好。但对晚期效果也不是很理想。考虑花费可能还是应在
国内治疗。 但若很有钱的话也可以到美国一试，星星推荐的两所医院是最好的了。

转移现在一般不开刀了。
过去一直是JOHNS Hopkins胰腺癌最好。
