先说一下值得记忆的好事， 有一个ID attending搞了个case study， 水平很高， 可
惜坚持了几周就放弃了， 可能是参加讨论的ID太少吧。 Rose的医学文献大家读是个很
好的主意，只不过因为太枯燥了，大家的兴趣不高。 为了娱乐大家，我们搞了医版的
非诚勿扰， liudier， lexian， chiusmd， sumo2009 等等都很出彩，让人难以忘怀
。 Dr.Newbie和dojo的争论也很夺人眼球， 不知道为什么newbie突然人间蒸发了， 令
人扼腕。 我搞了几次征文， 得到了againstwind， Rubyheart， forestpark1，
medwater， hims等的积极支持， 在此向他们表示感谢。 特别值得一提的是后期
Rubyheart的参与， 她的热心和才智，为本版增添了光彩。
负面的就是圣诞节期间某些外来ID的捣乱， 感谢futurehw的正确处理。 另外就是天牛
教授在本版的招亲实践， 实在让人觉得俗不可耐。
下一步我们版该怎么发展？ 我一直以为应该有一个新版主站出来， 带动大家上一个新
台阶。 我这个念头永远没有变， 希望有能人自告奋勇， 我会全力支持。
今后我们版要更强调临床实践， 大家畅所欲言，像美国同行那样交流心得体会。 今天
Dr.adropofsun的问题提得就很随意， Dr.Angie的回答也很professional。 这就是我
想要的。
BBS谈论了足够的种族歧视，男女CMG的差别， interpersonal skill， 等等等等。 可
我觉得我们CMG立足美国社会最根本的是靠本事， 是医疗技能，是丰富的知识加熟练的
运用知识的能力。
本事才是硬道理。
如今医学知识日新月异， 病人社会医院对医生的要求越来越高，我们在学习工作中经
常遇到各种新问题， 我们完全可以像adropofsun一样把问题提出来， 像angie一样的
医生会热心的回答你； 考版大军都是国内的医生， 遇到的临床问题一样可以提出来讨
论。对我来说， 回答问题本身就是自己学习的机会。 三人行， 必有吾师。
趁此机会， 我问一个问题， 今天让我琢磨了好久， 还是不得其解，不要让我失望啊。

discontinued after admission.

说大话谁都会，你到版上这么长时间， 没有看到你的一点实力， 连泡妞最基本的都不
懂。
这个版上的attending每年交的税相当于你的工资， 跟你信不信无关。

Unusually high levels result from liver disease, congenital enzyme
deficiency, or drug interactions which result in metabolic interference. The
patient with large variations in phenytoin plasma levels, despite standard
doses, presents a difficult clinical problem. Serum level determinations in
such patients may be particularly helpful. As phenytoin is highly protein
bound, free phenytoin levels may be altered in patients whose protein
binding characteristics differ from normal.
Most of the drug is excreted in the bile as inactive metabolites which are
then reabsorbed from the intestinal tract and excreted in the urine. Urinary
excretion of phenytoin and its metabolites occurs partly with glomerular
filtration but, more importantly, by tubular secretion. Because phenytoin is
hydroxylated in the liver by an enzyme system which is saturable at high
plasma levels, small incremental doses may increase the half-life and
produce very substantial increases in serum levels, when these are in the
upper range. The steady-state level may be disproportionately increased,
with resultant intoxication, from an increase in dosage of 10% or more. |

根本没有pensions. 这个世界上比我挣得多的人工作比我轻松的人，都没有几个。。。

我觉得就两种情况：
1. Pharmacy mistake
2. 病人自己吃药
现在两者都不承认。
药物interaction我就不知道了， 入院后停了Prevacid， 加了topamax。

retention give that type of delayed reansorption picture? Is this an OD
attempt?
Very interesting. Dilantin has high protein binding percentage and probably
very high body distribution volume. Did I'dt ever come down?

constipated since admisson.
OD is unlikely unless a pharmacy error.

something opposite.

teenage girls presented with their first seizure episodes right after the
onset of their puberty when I did my child Neurology rotation in last year.
In addition, it is not uncommon to see “relative” overdosing of
medications in anorexia patients or patients who have lost a lot of weight
in a short period of time.
I am not a Neurologist and I have no intention to step on your toes, so I
deleted my previous posts. Thank you for sharing this interesting case. |
