s******v 发帖数: 477 | 1 I admitted a patient 5 AM this morning. 40 AAF, c/o nausea vomiting and
extremely lethargic at home, found hypoglycemia glucose 35 by EMS.( nobody
has DM in the family, less likely she got any hypoglycemic agent ). She had
multiple similar episodes before. It always happened during her menstrual
period. Patient was found hypocalcemia since her 20s.
Ca 5.3, prolong QT 550 ms, Magnesium 2.1, albumin 3.7.Ultrasound found left
thyroid nodule, extensive bilateral basal ganglion calcification on head CT.
other labs were within normal range.
I ordered ionized calcium, insulin, c-peptide, PTH, TSH, thyroid scan,
cortisol levels, don't have result yet.
Any idea what might be the cause? | A*******s 发帖数: 9638 | 2 There are reports about Fahr's syndrome and hypoparathyroidism. But I don't
know anything about hypoglycemia associated with Fahr's sysdrome. Maybe her
GI symptoms caused her poor appetite which got worse during the period. So
hypoglycemia is just a different animal. |
