spinal tap， 第三天night shift hospitalist告诉我MRI是normal，spinal tap不能
做， 因为病人on fragmin， 要等24hr。
我自己看了一下MRI， 这个病人有spinal abscess。 当天就手术了。
这个radiologist的误读让人很恼火， 可是这个fragmin更是火上浇油。 我不明白，
现在的hospitalist都用fragmin for DVT prophylaxis? Why not SCD + heparin for
high risk patients? 这个病人根本没有on SCD. |

This guy was just recently admitted for a GI procedure complicated with E.
Coli bacteriemia. He was dicharged on ABX 3 days ago. Apparantly the E.Coli
just seeded in the spine.

trust is yourself.

you definately need to point this out to help them. you don't want this
double miss to happen again. this radiology one might be a human error, but
hospitalist one is a systemic one.

been and read at night. Sometime they are read by nightowl somewhere in the
world. Recently, they missed a perforated gut and perforated esophageal on
my patients. Scary.

prophylaxis更有效， 而且比lovenox便宜。 当然现在lovenox也便宜了。 医院的DVT
发生率高于10%， 所以强效fragmin是医院的standard of care。
想起北大医院李大夫的case， 我深刻理解他们的苦衷。 不过他们表示重新review一下
policy， 会认真address我说的问题。 |

？ 以前从来没这种事。

发帖数: 407 | 18 24 hrs is BS. 12 hrs for LMVH is reasonable for LP. For UFH, it is between 4
-6 hrs. There is not enough evidence suggesting Deltaparin is superior to
Lovenox or UFH for DVT prophylaxis in medically ill patient. I prefer to use
Lovenox 40mg daily, it is probably cheaper than UFH because of less nurse
work and supplies. Patient also has less needle stick.
