Applicant MUST identify his/her sponsor(attending) to be eligible to apply.

All applicants
1. this is hospital policy, so supposedly applies to all specialties
2. they have to search their own ways to find a Case attending to
sponsor them
3. they shall contact the number provided above, and request for the
application form (the attending needs to fill out the form to
demonstrate sponsorship) |
