1 case. A biopsy of transverse colon only contains piece of fat with some
cautery/crush artifact. What will you do?
2 case. A biopsy of colon polyp has a fragmented typical tubular adenoma.
However, in the early afternoon, I got additional slides for the same case
with the same label! Now I have two slides with identical label for one case
. The worse: I'd write up diagnosis. What should I do? |

will you answer?

s*********a 发帖数: 336 | 6 I think this kind of issue really put the pathologist onto a tough spot. I
agree that double check our own part first is a good idea. If things don't
change, I guess we might have to be honest to the surgeon/gastroenterologist
or even the clerks about what we've found here in the lab, and tell them
that we have already double/triple checked. Plus I would offer any help I
could to fix the problems because I don't want any of my colleagues to look
stupid in front of our patients. Dr. Jonzhang, please advice. |

A. Go check block against slide/
B. Go check the container including the lid(!). And filter the contents into
another block (wrap with filter paper or mesh bag).
C. If the block matched with slide and the remaining contents does not have
anything meaningful, call the MD to tell the situation and ask for his/her
input. In fact, he said "Oh, I thought it was a lipoma and thank you for
double checking everything." My problem kinda resolved.

with the same label! Now I have two slides with identical label for one case
. The worse: I'd write up diagnosis. What should I do?
Again standard QC protocol:
A. Check slide against block. => It turned out the part has two blocks, but
the lab didn't log in and just had one on file (hence duplicate of part I-1)
. So this is an easier case of possible error.
