Alcoholic liver disease:
- Alcoholic steatosis (Fatty liver)
- Alcoholic hepatitis( mallory bodies, neut, infiltration, perivent, inflamm
, liver cell necrosis)
- Alcoholic cirrhosis/fibrosis
F atty liver, alcohol hepatitis , and even early fibrosis can all be
potentially reversible if alcohol intake is stopped except late stages of
fibrosis and cirrhosis.
True cirrhosis (with regeneratrve nodules) is irreversible, regardless of
alcohol abstinence
Alcoholic fatty Iiver :
- most asymptomatic; minority mild tender hepatomegaly. Develops in 90~
100% heavy alcohol users.
- can develop after a single large binging episode , and does not predict
progression to hepatitis , fibrosis, or cirrhosis unless alcohol intake
continues to be substantial. 80~100% binge or heavy drinkers present. 15~20%
develop alcoholic hepatitis, 50% of those individuals going on to develop
cirrhosis.
- His: excessive accumulation of fat, most prominently in the centrilobular
zone. Macrovesicular steatosis. The hepatocyte nucleus is displaced
eccentrically by one or more large fat droplets.
Alcoholic hepatitis :
- Acute onset RUQ pain and modest fever, hepatomegaly, jaundice , and
anorexia.
- AST/ALT >2 ( 2nd to pyridoxal-6-phosphate , a cofactor for ALT enzymatic
activity); AST and ALT are always < 500 IU/L ( if higher transaminases,
suspect concurrent hepatic injury 2nd to viral hepatitis, ischemic hepatitis
or acetaminophen usage). ↑AST, ALT, AKP and rGT.
- Liver biopsy : ballooning degeneration, PMN cellular infiltration,
fibrosis, and necrosis; mallory bodies in severe cases. Accumulation of fat,
protein, and water within the hepatocytes→cellular swelling →necrosis; Nc
, Lc and Mф pervade the parenchyma.
- Tx:
• Abstinence from alcohol
• Nutritional support
•Corticosteroids (e.g. methylprednisolone)
Alcoholic cirrhosis
- an increased ratio of AST/AL T, elevated alkaline phosphatase , jaundice ,
and hematologic abnormalities in conjunction with the stigmata of chronic
Iiver disease. | S******9

Normal physiological changes of pregnancy: mild ↑AKP ( placental secretion
of the enzyme), normal bilirubin, AST/ALT and r-GT.
Acute fatty liver of pregnancy:
- Cx of the 3rd trimester of pregnancy
- asymptomatic , or complain of severe nausea , malaise , headache , and
abdominal pain; Over half of patients also have preeclampsia, and 60% will
develop acute renal failure.
- can also cause a prolonged prothrombin time and moderate to severe
elevation of transaminase levels.
- His: microvesicular fat deposition in the hepatocytes without evidence of
inflammation or necrosis.
Intrahepatic cholestasis of pregnancy (ICP):
- functional disorder of bile formation in 2nd and 3rd trimesters of
pregnancy.
- intense pruritus significant on the palms and soles and worsens at night;
and ↑serum bile acid
- 0.2~4% incidence, highest in Bolivia or Chile.
- Tx: ursodeoxycholic acid; hydroxyzine and cholestyramine are alternative
therapies
- prognosis: resolves shortly after delivery, may recur in subsequent
pregnancies and increased risk of developing gallstones.
- Cx: fetal prematurity, meconium-stained amniotic fluid, intrauterine
