按照新版表格填表说明：
Answer all questions fully and accurately. If a question does not apply to
you (for example, if you have never been married and the question asks, “
Provide the name of your current spouse”), type or print “N/A,” unless
otherwise directed.
那么我没有middle name，应该空着还是填N/A?
还有other names you have used，如果没有，我应该全部填上N/A还是空着就好？
