2. Check of $1,225.00 payable to U.S. Department of Homeland Security
3. Copy of PrimaryApplicant’s I-140 Approval Notice;
4. Copy of PrimaryApplicant’s former and current Passports and Visa;
5. Copy of PrimaryApplicant’s I-94;
6. Copy of PrimaryApplicant’s I-20 forms;
7. Form I-693, Medical Examination, for PrimaryApplicant;

10. I-485 Supplement J, Confirmation of Bona Fide Job Offer, for

14. Recent three months paystub of PrimaryApplicant.
For Derivative’s Adjustment of Status Application

1，来美国只有2年，只提供2年的W-2可以吗？
2，如果还有小孩子一起，需要每个孩子填写一个package吗？
3，从来没有过免疫记录，怎么办？重新打一边？ |

发帖数: 63 | 16 我可以回答你的第三个问题，去你的primary care那里，把USCIS要求的那个表给你的
医生，让他给你查，看疫苗有没有抗体，如果没有就在primary那里顺便打了。之后要
求你的primary care给你建立一个immunization record，这个以后孩子上学也用得到
，拿着这个immunization record再去找移民体检医生，这个应该可以省你很多钱。因
为疫苗在primary care那里打是医保cover的，移民医生那里打都是自费的。
