I can do this for most pages but not all. For example, on the first page of
each chapter (and also acknowledgement, index etc.), the page is placed
automatically in the bottom center. How can I change it to top right?

chapter or \section will automatically overwrite your personalized page
style. So you should redefine the page style in first page of each section
and chapter.
