"I swear I waive the YHP prescription"
and in next section, check "1 year",
and sign your name.

When you go to check the health before going abroad in the specific hospital,
at the time it is your turn to register,
you tell the receptionist that you not only need to check,
but also need to fill the university's form which you are carring.
Then s/he will take over/look at your form,
and tells you you must pay additional RMB for it.
