provide a copy of the Declaration's Page from your new Insurance Company
showing the effective date of your policy. This information is requested to
ensure you do not have a lapse in coverage. This can be faxed to 305-356-
5881 mailed to the following address. Once this information is received, we
will adjust the cancellation date of your policy.
