If there is a trump loser, we need spades 3-3 to discard 4 hearts.
If both follow on DQ, we concede a spade immediately. Then use 2nd trump to
dummy to ruff 2nd spade to set up the suit, and 3rd trump to enjoy the 4
spade tricks.
BTW, although 6D looks good, I don't like either 5D or 6D bid.

To set up 4 spade tricks, the best chance is to concede a trick first to
avoid possible overruff. This will give up the overtrick if spades are 3-3,
but it seems the only way to make if LHO has 2 spades and 3 trumps.
