1. I am aware that, if I make any false statement in this Affidavit, I can
be prosecuted criminally under Article 210 of the Penal Law of the State of
New York and I am making this Affidavit in support of my request for a
reduction of a certain Vehicle and Traffic charge as stated below.

There was personal injury or death sustained by anyone involved.
I am represented by an attorney. Your attorney must submit this reduction

第3个问题怎么填.COURT在TICKET可以找到.但JUDGE是谁呀?
第四项'I HAVE been convicted '是说我已经被定罪了?
什么时候寄这封信? 在寄告票回法厅之后,法庭回信之前.还是等上庭之前?
