First, I have to send an initial application + application fee ($125) and
examination fee to the CPA examination Services.
AND do I have to send the application for licensure and first registration (
FORM1) at the time when I first apply for CPA exam? I'm confused because I
though FORM1 is needed ONLY when you have passed all four parts of the CPA
exam?

You need to pay the $300+ licensure fee to sit for the exams in New York.
