转，凌晨老是痛醒。接着看了一个月Chiroprator依然不见好转。Chiroprator让我去看
Physical Therapy。Physical Therapist让我做了一些稳定腰椎和锻炼腰肌的练习还是
不见好转。后来做了MRI。MRI报告说：
1. Mild degeneration at the L4-5 level includes a small disc bulge and mild
bilateral ligamentum flavum hypertrophy. No lumbar disk herniation is
evident.
2. Very mild T12 and L2 level vertebral body compression deformities of the
chronic and benign and/or of doubtful clinical significance. No acute

发帖数: 5094 | 20 还是概念清晰，诊疗明确吧，另外是不是对病人做出了最好的决定，还是图省事，打发
了就算了。
举个小例子。外院转来个病人，头疼，腰穿怀疑是SAH，CT head啥都没有，做了
cereberal angiogram，发现一侧vertebral artery disecting aneurysm。
看外院的angiogram，那个aneurysm离PICA挺近(想想wallenberg)，intervention
neuroradiologist有点不太想做coiling。病人这两天觉得头疼变好了，不如送回家观
察算了?
结果病人第二天早上说后背疼.attending的hypothesis是因为血向下流，所以头疼好了
，该成后背疼。重复腰穿还是继续出血。于是决定做coiling。可是aneurysm填上了，
病人整成了wallenberg怎么办？他们让病人在清醒的状态下做angiogram，用ballon做
测试，就在cath lab做neuro exam，确认没有问题后做的coiling。那个手术从早上一
直做到了下午.

Unilateral carotid stenosis just does not do the job -- You just have to
have both sides of the brain out to make that person unconscious. If you
have to pick on stroke in this case, think about vertebral-basilar problems.

acute? How is the blood flow in vertebral arteries?
My impression is that the syncope was due to brain stem infarct rather than
SSS. SSS is probably an incidental finding.Having said that I haven't seen a
case with retrograde basilar flow from SSS(just realize our hosp doppler
ultrasound never reports basilar :( ）

MRA doesn't. As long as there is flow in a vessel, retrograde or ante grade,
it will light up with contrast enhancement. In time of flight, retrograde
flow will lose signal and the vessel would be dark.

just a part of it.
2. Not sure about vertebral A direction, I would assume it would be
antegrade on the right and retrograde on the left.
