外一只猫曾经有过UTI，吃过一段时间处方猫粮，2猫一起吃的。当时不止一个医生和我
说没有病的猫也可以一起吃，总觉得不太对，那个猫粮成份又差，吃了2个月就停了，
难道是这个给吃坏了？这都是4，5年前的事了，不会是从那时候就CREA高了吧。 |

回顾从step1自己闭门造车考了只带一个9的分数， step2开始参加小组学习，做了所有
能做的题只为拿个99，虽胸有成竹且考试感觉很好，最终但又得到一个9的分数。虽然
这次成绩不高，依然很高兴，毕竟不用发愁如果没过的话住院医开始后没有时间复习该
咋办。过去的3年中得到太多人的帮助 (此处略去人名20个)，绝大多数甚至从未谋面。
今天发贴也是为了回报我们这个社区吧。
自我介绍：毕业>15年，220+/220+/1st/210+；
复习时间：6月，头4个月每天约2-3小时，周末由于陪孩子们玩（前两年欠的太多，你
懂得）及做点家务（再不做但心被老婆炒掉），反而复习不了多少；最后2个月每天4小
时，鼓励自己考了这个2年内再不用受这个罪。
复习资料：随大流，MTB 3, UW CCS, UW step3 Qbank, UW step2 Qbank;
模拟：考前2月，NBME 1 190+, 考前1周，NBME 2 220+;

1. UTI: 如果culture sensitive to cefazolin, 决定给keflex，是 500 mg BID 还是
TID OR QID for 7 days or 5 days？今天一个PA说她学的是QID. 但uptodate 和
micromedex都是BID,快速看了idsa uti guideline，没提到keflex dosing，只有
提到证据不充分，也不知道是resistance problem 还是 lack of efficacy...
2. 今天进来那个巨臭的人，在床上躺了半年没人管，背上ulcer流得稀里哗啦，一想到
就噁心。。。anyway，admitted for hyperkalemia, renal failure, sepsis,
hyponatremia. 然后呢，allergic to penicillin, cipro. 病人无法说话，不知道具
体是啥allergic reactions. 在ED就得上ABX了，preceptor就问我啦，该用啥呢，我觉

2. penicillin allergy cross-reaction with cephalosporins and
carbapenem----概率不是很高啦，只要病人对pcn不是anaphylaxis，试一下cefepime也
无妨啊，如果allergic反正在医院可以抢救。aztreonam超贵的，而且只cover gram
negative,现在为了省钱自然就不用啦
cefepime coverage和zosyn差不多啊，除了anaerobes少了很多，你要cover anaerobe

我现在30+，女，八年前得过数次UTI，后回国查出尿潜血1+，还有两次尿检尿蛋白1+。
看过中西医，中医认为是肾病早期，开了很多补肾的药，没敢吃；西医认为没有严重的
浮肿症状，就没什么，把我打发了。
回美后做了膀胱镜（应该不是尿道镜，感觉探头没进入那么深，一下就做完了），没有
结石，肾部B超也没结石。从那时起，所有尿检都有潜血1+，跟月经期没关系；某次潜
血达2+，还有少量leukocytes，另一次尿检还有trace ketone。。
13年上半年做妇科B超时，B超师发现我虽然上了厕所觉得排空了，膀胱里还有些尿，这
是尿滞留吗？13年下半年做体检B超，发现右肾9.5厘米，左肾10.5厘米，没有肾积水，
这边的医生认为正常。

其实膀胱这是一个很常见，并且很费钱，很痛苦的疾病。这是因为排尿是一个复杂的反
射过程。一般的排尿障碍主要是脊髓损伤导致。但是脊髓损伤确非常常见(neurogenic
bladder)。胚胎／先天发育异常(spina bifida脊柱裂），脊髓创伤（比如车祸、战争
中的枪伤或者爆炸伤、事故（比如跳楼、攀岩掉落）、暴力事件（刀具等的损伤）、医

现在的情况就是，如果没有家属，老人住院如果没有SUROGATE/POA，就由医院的ETHIC
COMMITTEE来办理，一般是州政府相关部门指定一个SURROGATE来决定老人的治疗。但是
SURROGATE往往不是那么负责或是专业的，一般就是医生怎么说他们就怎么做。
有些老人可能就是TIA/UTI导致的暂时的MENTAL STATUS CHANGE，但是因为没有家属，

内科联合用抗生素药非常普遍，而且用严格的PROTOCOL，

home hospice或是nursing home hospice。我确实见过一些一周还没死只能转走的病人。
医院里hospice 病人其实癌症并不多，很多是重症感染病人。
比较常见的过程是人老了不能自理病重住进医院，医院治疗后把你送到nursing home，
但是因为nursing home条件不行，环境也比较脏，很多病人很快就会因为感染回到医院
。女性病人uti是很常见的，因为一般一个班才给你换一次尿布。
如此几个反复，感染越来越频繁，越来越严重，终于有一天你进er时昏迷需要气管插管
。气管插管是暂时的措施，如果两周内你还不能拔出管子自主呼吸，这时就有两个选择
，一是tracheostomy，一是hospice。
如果你的family不想让你更痛苦，很大可能你就被送进医院的inhouse hospice。
在那拔管。这种病人大多数拔管后很快死亡。

doctor decided is to put her on antibiotic all the times. Once she was on
antibiotic for 6 months, the doctor thought they can stop it, one week after
they stopped it, the UTI came back again in a very serious form. And they had
to changed it to another antibiotic. She had this problem after she came to
US, they think it's sth to do with the food here.
