An update: My Mom had a DDDR type pacemaker installed. She is on low dose of
metoprolol (25mg daily). As far as I understand, medicines for afib do not
eliminate afib except for slowing down overall heart beat. Is it right? It
is a miracle that her afib also goes away (from the telemetry exam) since
her procedure a week ago! Is it known that a pacemaker sometimes can help
improve afib?

cloud but we didn't have a choice. Sunset ridge is just Liberty ridge, it's
nearly impossible to retreat.
Wind was quite strong. From the recorded telemetry data, it was around 30mph
at camp muir, so I think it was 40-50 near summit. I was nearly blown off
my stance several times.

要收治入院在telemetry unit, 除了详细的问病史和体检，还要作syncope workup, 包
括1）排除心脏的问题， 如心律失常，心脏瓣膜问题，心肌肥厚，心梗等，做一系列的
心肌酶谱和心电图，做心动图echocardiogram, 做holter, 如果需要还要做stress
test, cardioangiogram冠脉造影等 2)排除神经方面的问题，如seizure,椎基底动脉供
血不足，脑溢血/脑梗等，做CT head, EEG脑电图等 3）排除其他的问题比如体位性低
血压，血管迷走神经vasovagal，脱水，药物引起的等等。
还要根据病人的年龄和既往病史等信息来综合诊断。
我们这里老年病人syncope特别多。很少有年纪轻的。
楼主的心脏科医生应该做了其中的一些检查了吧。 |

发帖数: 658 | 13 很象病窦综合症中的一类叫tachy-brady syndrome, 就是窦房结，或是房室结出现了传
导的问题。就前几天，晚上值班的时候，抢救一个病房里的老太太，found
unresponsive, called pacemaker team, telemetry strip showing tachy-brady, 也
是我第一次看到这样的病人。
建议赶紧去看心脏科医生，做检查。

Circuit 加damage 和clip
这一把枪就够了，远近通吃，chaingun子弹容量大，一群敌人不需要reload就消灭光了
，有了这把枪以后基本上是cake walk了
另外感觉Acid bath比Statis coating有用多了，
lower换grenade launcher也不错，加safe guard以后对着地面轰就行了，敌人无法近身 |

at 09:00 hrs Indian Standard Time (IST) this morning (October 23, 2008) when
the spacecraft’s 440 Newton Liquid Engine was fired for about 18 minutes

我碰巧也是作MEMS的。看来在美国学机械的中国人，很多都学Mems。估计大家喝我当年想
的也一样，沾点电子，今后好找工作，再不济也能作process engineer,呵呵，现在发现
这个想法是多么的幼稚。
首先念硕士，当时我是拿的fellowship, 找了一个老板作MEMS的，结果被安排了一个项目
是用MEMS sensor的application, 基本上是个system level的design工作。这种项目呢，
说实话,商业的色彩很浓重，作个Master project还可以，真的要做phd project 我绝对
很难。真的要做，也要到计算机系的老板地下去做，作embeded system or sensor
network or telemetry.
一年搞定硕士，我死活就转了，换了一个老板，还是作MEMS.说实话，到我转老板之前，
我对MEMS的理解还少的可怜，一方面是我原来的老板决定的，他也是半道出家；令一方面
我做的项目实在是个application，与mems没什么关系。这次我挺幸运，学校恰好来了几
个搞mems的导师，其中有几个还是 |

finished. 一年总共13 blocks, I will have my vacation(4 weeks) in Nov:-)
现在每组两个intern, 一个senior,每个intern大概10-12个病人. 每个病人基本上都是
不同的PMD,然后每天或每几天就要和PMD联系,要么就是PMD来病房,看病人,你正好在,碰
上了,能问问,比后来读他们的手写的notes好,notes真是天书啊:-)
我们这里每天早上7:30大家才来,print out the census,汇报给chief,自己有多少病人
,然后morning report,chief把头天晚上night float的病人给尽量均匀的分给not on
call的intern们.
每个intern每周都有两个下午要去clinic,所以要去clinic的这两天,是很忙的,早上交
班,有时候有讲座,上午又有个1小时的telemetry,ICU round,中午有noon conference,
下午1-4点去c |

发帖数: 272 | 33 不好意思让大家误会了。我们这里每个值班的第三年住院医都叫Chief,但是还有另外的
管行政的CMR,我不是那种CMR。
关于血糖，我们内分泌的attending是说如果病人NPO,那么长效胰岛素就减半。但是具
体问题具体分析。我自己的血的教训是这样的：
我以前有个病人是CKD stage 4,她第二天早上要做胃镜和肠镜，所以NPO,D5 1/2NS @
50ml/hr when NPO, 她原来用NPH 20units BID。我给减了半。早上护士就给了10units
NPH，但是病人不久就出冷汗，浑身颤抖，发软。血糖50。赶紧让她喝了果汁，推了
50ml D50,血糖还是上不去，推了两次D50血糖勉强100。我被第三年的骂得半死。
同样这个病人，有一次路上昏倒又被送到医院，又是我收的，病人因为是Syncopy被收
到TELEMETRY,rule out arrythmia and ACS。但是其实病人HB 只有7.5，她没有心脏问
题，昏倒是因为严重贫血，加上刚来过月经。结果我一心扑在她的Rule out arrythmia

My position is called transitional nurse, meanning training. After 8 weeks-12
weeks training, the title will be level 1. I do not know how long to promote
to level 2 in our hospital or unit. Just know there is 8-9% salary increase
when being promoted to higher level. |

送到医院，要不然出门了这些人就是"累赘"了。
另外，不同的科室会有不同的情况：我们医院的cvicu说他们放假的时候轻松，因为能
择期手术的都等到假后，除非emergency的手术；有的科室因为sensus低就要关门，把
残留的病人转到其他科室，最恨这些乱七八糟的病人了，药物不熟悉，医生也不脸熟，
discharge instruction也不清楚；我们科室比较忙，因为关门的科室会转病人过来，
因为我们的病床全是telemetry的，所以不可能关门，转过来的病人可以non-tele，但
是我们的病人都是tele所以没办法转到non-tele的floor |

with them. I haven't found out for sure if they sponsor green card or not.I
asked nurse recruiters, and they don't really know. I will let you know when
I hear back from them again. Those are all NICU positions. Are you thinking
to find a job in other cities or you just want to stay here?
