体，一个pd-l1 抗体。 在melonoma, lung cancer, renal cancer 上效果都不错，秒
杀过去几十年搞的那些药，如果这都没什么意义，癌症治疗也没啥搞头了。目前看到的
肿瘤生长曲线已经非常的exciting了，有兴趣的可以去读读这三篇文章，看看肿瘤曲线
。从ctla4 到pd-1/pd-l1，不仅仅副作用下降了，疗效也有了大幅度的提高，再等3年
，让我们来看看pd-1/pd-l1 的长期存活率，那时候会更有意义。
anti-ctla-4, 具体可以参考nejm上hodi 的Ipilimumab文章。 在50％存活率这个点比
较，你会发现这个药的疗效也一般，但是重点不在这里，大家可以比较2年半以后，也
就是30个月以后，这个存活曲线的走向。 晚期黑色素瘤,长期存活率提高15%, 10个人
里，可以救活1.5个人，现在有那个药可以做到这点？我觉得这就是重大突破。

我在Holton实验室的时候，尽管组里有20多名博士后，但已经好几年没发表什么论文了
（好像98年有一篇Tetrahedron Letters），也不用申请课题，因为半合成紫杉醇的技
术转让给Bristol-Myers Squibb（BMS）公司后，他获得的转让费太多了。我去那年就
知道BMS给FSU每年1600万美元转让费，而Holton和学校是1:1分帐，当时他已经拿了三
年的转让费，也就是说2400万美元了。我的研究结果前后申请并获得了三项美国专利，
但可以发表论文的部分被压下来了。我不是Holton实验室第一个也不是最后一个这样命
运的博士后，所以也就不去抱怨了。 |

前段時間，在美華人界的有個大新聞是住在紐澤西州的華人李天樂用放射性元素鉈(
thallium)毒殺自己的丈夫，其中有罪無罪是是非非，並不是中醫關心的重點。但是在
這個事件中，醫易堂注意到一個有趣的地方：據新聞報導稱李是在美國施貴寶製藥公司
（ Bristol-Myers Squibb）上班，她的工作讓她有機會取得這種劇毒的元素。到底鉈
是什麼東西，不清楚的朋友可以查閱一九九四年清華朱令中毒事件。
那麼我們這麽想，製藥公司為什麼有這種劇毒的物質，難道是放在那做展覽品？當然不
是，說穿了就是製藥公司用這種放射性元素做研究並且製藥。怪怪，這種劇毒
的物質拿來製藥，別人不說，醫易堂看了都覺得毛骨悚然。
第一，我們不知道到底製藥公司拿這種東西來做什麼藥，姑且先不論對症的情況，如果

Hospital pharmacy carries the responsibility of finding the drug that is
approved and on the market.
It will be highly unusual and unethical for US doctor to prescribe
chemotherapeutic drug to an unknown patient. |

美国制药商百时美施贵宝（Bristol-Myers Squibb）称，公司将裁员4300人，并
且关闭27个制造厂中的一半，这两项举措将为公司节约15亿美元的支出。
作为重组计划的一部分，公司还将在其产品组合中削减成熟药物品牌产量的60%。
这些措施将于未来几年内陆续执行。此次裁员人数占公司总员工数的10%。
由于一些重要药品的专利使用权到期，最近几年公司业绩不佳。
但是公司新药，如治疗精神分裂症的药物Abilify及抗癌药物Erbitux，销量的增
长将提振公司未来盈利情况。

for generic drug companies as of April 2013 is 5.4 percent. The largest
average profit margin is for major drug manufacturers at 18.4 percent. This
group includes Pfizer, AstraZeneca and Bristol-Myers Squibb. For the "other
drug manufacturers" category, which includes Teva Pharmaceutical Industries
and Allergan, the average margin drops to 12.2 percent.
