发帖数: 658 | 2 是的， 类风湿关节炎是Rheumatoid Arthritis。 3线药中，只有对症的NSAIDs可以在
药房买， 其他要医生处方。 看报告crp和esr有些高，显示可能有活动。控制病情的药
DMARD要看病人的情况来决定2联还是加生物制剂。 病人不在美国，医生不会给开药的
，因为免疫抑制剂有非常大的副作用。
另外，类风湿关节炎现在强调早期用DMARD。如果不上，2年内手指erosion的机会非常
大。 建议在国内治疗。因为国内很多医院有风湿科，可以看类风湿关节炎， 比如上海
仁济，北京协和。 |

国内朋友两岁的孩子不幸得了这个病，非常着急，希望能得到些建议。
孩子9个月大时就出现了关节肿胀，由于没意识到问题严重，11个月时才开始入院，现
在一年多了。
目前的症状是多个关节出现肿大，不发病时可以走路，但情况非常依赖药物（Naproxen
），不吃药的话睡觉都会蜷成一团，十分可怜；总吃药的话，孩子很容易感冒发烧，有
时高烧不退，非常危险。
早些时候用过抗风湿药副以少量激素， 效果差，副作用大，才改用生物单抗类药的。
想问问这种病可以长期用药吗？有没有比较好的治疗手段？国外尤其在美国市场有没有
好点的药，能不能直接买到？

类风湿给的解释似乎是真正的rheumatoid arthritis,而风湿性关节炎虽然也
叫rheumatoid arthritis, 但是居然撤上了中医理念，还联系上了风湿热
（rheumatic fever),会造成心脏瓣膜的损伤。这可是比rheumatoid
arthritis还要罕见的病啊，至少在美国是这样。
这个问题的根源在于将rheumatic这个词翻译成了风湿。西医里面的rheumatic
指的是免疫类疾病，和中文字面上的风湿没有任何关系。
其实在我看，国内大多数人的理解是只要是慢性关节炎，尤其是阴雨天更严重的关节
痛，都是风湿性关节炎。这个是严重的误导。关节病的病因不下几十种，多数人都有

, polyarticular joint pain/stiffness, dry eyes, negative rheumatoid factor,
but i don't know what is 强直性脊柱 in english name,

then i would think maybe sjogren syndrome, primary sjogren has nothing to do

different in patient with and without Sjogren's syndrome. But it is clear
that Sjogren's patient has a high risk for rheumatoid arthritis. My
understanding is that the treatment will be the same on the arthritis part

节炎 very liberally, including pretty much all types of joint pain.
Rheumatoid arthritis is serious, but lots of treatment are available. Most
of them available in China as well, at least in big city academic hospitals.
She should see a rheumatologist.

go without treatment for ten years. Rheumatoid arthritis causes
irreversible damage to the joints, the outcome far more severe than the
potential adverse effect from treatment, including well monitored

only one of the many options. There is not a single qualified evidence

clinical diagnosis without any positive labs. I doubt your doctor is
convinced you have rheumatoid arthritis because she didn't start you on
specific medicine for RA.
Did your doctor tell you what was found in the fluid from the knee, crystal
etc? It usually can give a clue.
What finger joints are affected exactly, the most distal ones or the
proximal ones?

回答你的疑惑：
1。这么多人都说他们的亲戚朋友已经完全好了
风湿性关节炎并不常见，大部分关节疼不是风湿性关节炎，好了自然不奇怪。
2。我爸爸当年被诊断什么白塞氏综合症（也是免疫性疾病），西医也是说没有办法治
疗，
也是用的同样办法治疗的，以后再没有犯过。
这是典型的Bechet病史，男性年轻时发病，一段时间后很多病人自愈。在发病时，其实
有很多药可以减轻症状。
3。你去问问得病的人，哪个不是一到阴雨连天的时候就病情加重？
大多数有关节痛的人都是这样，风湿性关节炎当然也不例外，但不表示风湿性关节炎是阴
雨环境造成的。这样的理解可能是因为中文里面rheumatoid翻译成风湿。而在决定这个
翻译时，医学对免疫病还一窍不通。其实rheumatoid和wind or water没有任何关系。
4。真是选择性失明
我还是建议你在医学问题上，在没有google之前，对所谓专家，中医和热心人的观点选
择性失明。
