excretion is the primary mechanism and increased plasma volume suppressed
renin release. But I remember there is 1 NBME Q asks about renin in
hypertension and the answer is renin, AgII, aldosterone all high. Which one
is right? Thanks!

loop diuretics will decrease blood volumn and decrease RBF which will
increase the activity of both renin and ACE.
ACEI will increase renin also by blocking the negative feedback.
I remember that ACEI is contraindicated if there are stenosis on BOTH sides
of renal artery.
I think if we dilate the obstructed left renal artery by percutaneous
transluminal renal angioplasty, the BP and renin will drop.

selective betablocking agents have adverse effects on
glucose metabolism, prolong hypoglycemia or mask

good by to USMLE
Material: MTB3 and UptoDate. Step 1/Step2: 250/249 but those were 3 years
ago. Step 3: 227. Time: on/off for 3 months.
Good luck to everybody.
Not a whole lot of thoughts about this test. if you can memorize MTB3 and UW
, then there should be NO problem to pass it.

发帖数: 248 | 27 我对这个药的市场占有率持怀疑态度。renin inhibitor走的是抑制“肾素-血管紧张素
”这条通路。对这个通路的研究，和作用在这个通路上的高血压药的开发技巧，在15年
前就是药理书上的经典章节。血管紧张素抑制剂（XX普利）已经在市场上很多年了，效
果非常好，再过一两年这类药的专利马上就结束了，而一旦“XX普利" 变成generic药
物，我看不到novartis 的Aliskiren有什么优势。这个药的oral bioavailability好像
是4%左右. |

其实被医生问的大多数都是跟药相关的，说两个吧
一个是direct renin inhibitor tekterna.很少有医生开这个药，却被我遇到了一个病
人用这个药，是primary care开的。hospitalist问我对这个药的看法，我居然连class
都不知道：（
查了后发现，这个药跟ACEI or ARB结合用是contraindicate to diabetic patient的
，FDA的编注是，有study表明这样的combo会升高stroke and death的几率。Valterna(
valsartan and tekterna) will be off market in this July. 在药房工作的同学们
可以注意下。
另外一个药是simvastatin。 非常常见hospitalist一上来就给病人用40mg. 我跟带我
的医生提出来： 20mg reduces 38% LDL, and 40mg reduces 40% LDL. 她是个非常随

对于diabetics/ppl with renal dysfunction不好。。今年年初开始普及。。。JNC8到
现在还没出来。。。
Simvastatin一年多前的rhabdo闹得沸沸扬扬，和很多药并用都要decrease dose了。
80mg都是not recommended。我觉得你降低了dose 很好，说不定病人还吃什么其他的药
increase simvastatin level-->increase rhabdo risk呢。。 |
