您把我的一夜难眠扩大化了，扩大到了极点。事实上，我就失眠了一夜，想买个好车，
跟个好姑娘去兜风，我觉得这是非常善良美好的愿望。。呵呵。
我经常在说，我有更高的理想，但是被你华丽的忽视了。我最快乐的事，是找一位
CMG Resident or Attending，然后一起为治病救人而奋斗。这里的人都忽视了我的理
想，以为我就想好车好房。I want to make sure that everyone understands my
feeling.

When EBM is against the reality, it is natural to question the evidence. I
agree with you the studies have to be re-designed. But in reality, how many
of us would question these results if no conflicted interest?
I make two examples here: one is the Lexian's kyphoplasty, please see the
link I provided in that thread. Another is cardiac angioplasty or stenting.
But if we believe in EBM, then seroquel for psychosis in PD would be off

symptoms, I would be more worried about the flare-up of psychosis when D2
agonists are used. So answer is No.
I am not aware of any clinical use of D2 agonists for parkinsonian dementia.

EBM之所以在美国流行无外乎以下几点：
1. 医学是科学，所有诊断要有科学证据才能令人信服。
2. 保险公司有章可循
3. 法院对医疗事故的判定有章可循
4. ICD code的推行，政府对医疗管理（regulation）有章可循
但是， 不可否认， 这个EBM有着不容忽视的缺点：
1. 很多证据或者数据的可靠性成疑，因为这些数据或结论或多或少受商业，政治以及
个人因素的偏差（bias）影响，使得相对一部分证据不能真正反映真实， 这方面的例
子不胜枚举，比方说一些手术指证，等等。
2. 询证医学的提倡普及，让医生们成为执行EBM的机器人，美国医生的创造性在相当程
度上被抹杀了。 客观上限制了医学的艺术性。 现在的EMR推广更是让这种情况雪上加
霜。
不可否认，EBM对医学的发展是革命性的，不能因为它的局限而否定，这也不是我的本

A大指教；late onset psychosis 可能性或许有，但不大， 请benpu指教。是不是应该
做个头部CT看看， vascular Dementia 或 atrophy。能不能查查TSH，b1，B12
，folic acid， ammonia，mg， 等。

deficiency so needs to be treated with dopamine agonist, hence it's a
dilemma to treat Parkinson Dementia or psychosis.
In this case, it's possible the Risperdal worsened the underlying Parkinson;
or pt has PSP or LBD, which has Parkinsonian symptoms but not responding
well to dopamine agonist, while 'super-sensitive' to antipsychotics .
Actually in these mind of cases, Seroquel is the drug of choice if an

1. PRITE: Psychiatry residents must take national PRITE tests yearly.
Usually the program coordinator will have the previous PRITE test questions

尽管宗教可以解释某些人的幻听，但对这个老年人出现幻听最合理的解释还是老年痴呆
。这种psychosis可以是痴呆的早期症状，而平常的门诊很难觉察出痴呆的发生，所以
不能怪garland。要知道现任的高等法院法官很多都有痴呆，里根总统在他当总统以前
就已经出现痴呆的早期症状了，13年后他才正式被诊断alzheimer's。

心理防御机制，是指人面对紧张情绪时避免精神上的痛苦和不安而采取的一种应对方式
。每个人的性格与经历的不同，所选用的防御措施也大不一样。
1.升华：
把社会中所不能接受的转为所能接受的，以保持内心的宁静与平衡。
例如，居里夫人年轻是曾恋爱受挫，对方因她饿长相和学识浅薄而嫌弃她，从此，她立
志在学术上超过此人，于是埋头发奋，终于成为举世闻名的科学家。
2.合理化：
潜意识里有一种似乎合理的解释来掩饰自己，其目的是减少因挫折而产生的焦虑，保持
个人的自尊。
例如生活中常说的：不打不成才，棍棒下出人才。病人把因疾病导致的明显体重下降解
释为“减肥”，等等。
3.补偿：
个人的身体和心理缺陷，通过补偿而得到非常有效的纠正.
例如，某些残疾者通过惊人的努力，克服自身的缺陷，成为著名的画家，运动员，等等

俺原来有一次空调不work，测了一下是compressor不work+缺氟利昂。换了压缩机，冲
了氟利昂，过了几天又不行了。发现氟利昂漏。后来干脆不休了。
