array/MS似乎还有很长一段路（希望aptamer array能迎头赶上呵呵）。小分子（
glycan
，metabolite，etc。最近出了个sarcosine）呢？lectin特异性太烂。希望aptamer
能顶上？不然也许只能指望HPLC之类能high throughput了？
其次得能把差别和各种physiological/pathological condition联系起来。这个ms进
展也太太太太慢了吧？就说cancer吧，有几个大家都服气的diagnostic/prognostic
/endpoint biomarker？一个PSA用了这么久，还一直被批。
最后要能根据这些差别来调节therapeutic regimen。Y/N型的或差别大的好办，学
herceptin
附赠个kit就是。检测分辨率不够的话呢？最好还是能够下一样的药，对不同的人达
到不同的效果，controlled release？也许就要在delivery method上做文章了？各

期诊断非常重要. 比如乳腺癌的治愈率高其实大部分应该归功于早期诊断. 而胰腺癌早
期诊断很难, 五年死亡率非常高.
在临床肿瘤学的研究中, 有一条非常重要的, 叫做stage-adjusted. 就是说, 比较的时
候一定要把诊断肿瘤的stage算进去, 不同stage的不能直接比较. 比如上面的五年生存
率, 如果都是在stage I发现, 那肯定高; 如果都是在stage III发现, 生存率就低很多.
上面争论的各位, 不管你的观点是什么, 基础知识还是需要的. |

岁，我收的时候他还好好的，冲我笑呢，就是sats低，lactate只有4。overnight就被
intubate拉，we did everything we need to do, 可他还是接着一天不如一天，挺了
一周，kidney开始down，platelets也开始drop，家里很快就决定withdraw care了。当
时还想着应该再给他一些时间。。。。
另外一个72，收得时候lactate7点几，一度11点几，ph7.0几，k7点几，fluids
,pressors, bicarb drip, dialysis。。。能用的全用了，当时觉得他肯定是不行的了
。。。。没想到一天一天好起来，前两天居然extubate了。。。就在我感叹年龄真是
icu病人的prognostic factor啊的时候，他还是tolerate不了拔管后的destress，面临
再度被intubate和long term weaning process， 刚上去看，已经决定comfort |

。而且各个诊所对什么FSH值对应normal ovary reserve的标准也不一样。我的诊所把
FSH<=12都归在成功率最好的一组。mm也去诊所问问吧。
下面这个link给了一个normal FSH level的表格。也给出了一些原因说明不能完全用
FSH值推断卵巢状态。
