本人有心动过速的病史，Atrial Flutter-病发两次后曾经心衰竭。做了个小小的
Abalation．从此就吃药控制心跳。Coreg 50mg. 吃了两年，后换成Metoprolol 50mg.
一直吃到现在，应该是一辈子就这么吃下去了。
问：有时候我胸口会疼，针扎的疼，一阵一阵的，像牙痛那样。做了彩超等等检查，没
看出什么不妥。有两个心脏医生说可能是伤口疼，nothing they can do. 你们觉得呢
？前两天疼了一整天，今天没事了。偶尔就会来这么一出，特别是身体太累状况不好的
时候。
再问：小腿总是小小酸麻，我也没走很多路，工作是坐办公室的。我只是缺钙吗？还是
可能是药的副作用。问过医生，医生说跟我的病没关系。我很纳闷。
再问：吃Metoprolol，能怀孕生子吗？因为我心跳太快，怀孕的话更加要吃
Metoprolol，因为孩子心跳快就会让我心跳更快，所以药是必须吃。有医生说可以，有

没问题，每个医院ALT/AST正常范围可能都有出入，一般是如果增加到大于最高正常值
的三倍才算肝功能异常
单用CK值来评估心脏功能太不准确了。CK-MB在心肌梗死发生前4小时才会增加，之后就
降低到正常。心衰是说heart failure吗？一般是查BNP
这个lab值在我看来心脏和肝脏都没问题，就算没有baseline lab，也没有明显疾病特征
2/ statin类降脂药与钙离子通道拮抗剂合用有强烈的伤肝作用吗？
simvastatin是研究最多的statin，至今FDA仍然要求跟calcium channel blocker合用
要用低剂量的simvastatin
跟amlodipine合用最高剂量是20mg
rosuvastatin的drug drug interaction比simvastatin少，不同的statin之间比较降血
脂强度，要看剂量如何，rosuvastatin 2.5mg 降血脂强度不如simvastatin 20 mg

如果病人因为高血压吃BETA BLOCKER，比如METOPROLOL，同时吃利尿药降压，就有可能
因为DEHYDRATION导致肾功能降低，这样METOPROLOL的代谢就受到影响，累积在体内，
导致心律过低。这在临床很常见。类似的还有地高辛，复方丹参滴丸啥的。
这属于可纠正的。
心脏本身的疾病，比如SICK SINUS SYNDROME就是导致心律低的常见原因。
病人就可能需要PACEMAKER了。
心律过低，如果同时发生2ND DEGREE/3RD DREGREE HEART BLOCK，就要注意了，尽快装
上PACEMAKER。否则有可能导致心跳骤停。
家里备阿托品急救。我们在临床上，如果病人心跳只有3，40，很多都要在床边放一只

为在国内每次看病有不一定能再挂到同一个大夫的号。 出了冠状动脉痉挛引起的心绞
痛，我爸还有前列腺增生，所以也在吃控制前列腺增生的药。 他现在每天吃一下4种药：
1） Aspirion Enteric-coated tables 阿斯匹林肠溶片
2）Amlodipine Besylate Tables 5mg 洛活喜
治疗前列腺增生的：
3） Prostat Tables 70／4 mg： （普适泰片：良性前列腺增生）

发帖数: 352 | 13 CCB应该是 diltiazem. 我觉得IV dig+Dilt+metoprolol 同时用,特别是PO switch to
IV,只要剂量对了，没什么不对。至少在cardiac ICU我见过多次。一般都有hold
threshold. 比如 hold metoprolol for HR < 60 之类的。
同意楼上，护士给这类药的时候也需要有clinical judgment, 毕竟药剂师不可能时刻
monitor每个病人的 vitals. |

Change to Toprol XL (metoprolol succinate) 100mg QD or 50mg BID. or Coreg
If she can't tolerate ACEI, she needs to be on ARB for CHF. ARB approved for

1.你说你妈妈年轻时候血压偏低，请问她现在的血压多少?
2.你妈妈能感觉出明显的a-fib症状吗，比如明显感受到心跳不齐。如果afib症状明显
，心跳维持在110以下也是可以的。不需要太苛求。
3.你说她吃了metoprolol以后血压过低，请问她吃的是哪一种metoprolol,剂量多少，
怎么吃的，吃药以后血压多少？需要警惕的反应是systolic BP<80, heart rate<60。
医生应该调整药的剂量，不是直接就不吃。。
我说的时常监控的是心电图。sotalol在4%的病人中会产生qt prolongation and
torsade de pointe(ventricular tachycardia)，这个副作用是和血药浓度正相关，所
以要结合你妈妈的肾功能来考虑。
最后，你妈妈的药你也买不了啊，没有处方。 |

通过LZ后来的描述，病人更像装的是ICD,每次电击后入院接受amio的治疗

发帖数: 352 | 19 1. AST/ALT 只是 mildly elevated, 不足以说明是肝功能异常。单纯CK elevation 也
和心衰没有关系。CK其实也不算很高了，我觉得这三个lab偏高主要是 rosuvastatin的
副作用 － LFT elevation, muscle break down. 其实CK也没有高太多，病人有没有肌
肉疼痛的症状呢？我觉得应该再查一下scr, 以及lipid panel。不过既然你有AST/ALT
了，说明做了CMET, 那creatinin 你没有列出来说明不高对吗？那说明还没有发展到
rhabdomyolysis。这个时候如果LDL不太高，可以暂停 rosuvastatin. 针对lipid
profile 选择合适的降脂药。
2. calcium channel blockers may decrease the metabolism of statins. but the

An update: My Mom had a DDDR type pacemaker installed. She is on low dose of
metoprolol (25mg daily). As far as I understand, medicines for afib do not
eliminate afib except for slowing down overall heart beat. Is it right? It
is a miracle that her afib also goes away (from the telemetry exam) since
her procedure a week ago! Is it known that a pacemaker sometimes can help
improve afib?

发帖数: 198 | 26 详情见下贴，今天从国内得到一个信息说很可能是病毒性心肌炎造成心肌受损，只是心
脏检查是两周多后所以病毒感染已经结束所以看不出来，国内一个亲戚几乎是一样的症
状及时地服用了西药：酒石酸美托尔片，此药是处方药，在中国的商品名：（倍他乐克
，Metoprolol Tartrate Tablets 。）十天左右失眠心慌就得到了很大缓解。而且这
种药的副作用不是很大。
不知道大家对此有何建议？
1。 没有明显的感冒发烧症状是否能排除心急被病毒感染的可能呢？多谢了。
2。 这个要有没有新的替代型可以使用？
3。 还有从开始心慌心悸到突然出现严重失眠，就是吃了医生的强效抗焦虑和抑郁的药(Lexapro)。是不是可能因为这个对心脏有很不好的副作用导致严重失眠突然出现呢（如下所述，吃的当天晚上两次心动过速，严重失眠也就此开始）？

状及时地服用了西药：酒石酸美托尔片，此药是处方药，在中国的商品名：（倍他乐克
，Metoprolol Tartrate Tablets 。）十天左右失眠心慌就得到了很大缓解。而且这
种药的副作用不是很大。LD从一开始就怀疑是病毒性心肌炎自愈后遗留的毛病。
不知道大家对此有何建议？
1。 没有明显的感冒发烧症状是否能排除心急被病毒感染的可能呢？多谢了。
2。 这个要有没有新的替代型可以使用？
3。 还有从开始心慌心悸到突然出现严重失眠，就是吃了医生的强效抗焦虑和抑郁的药
(Lexapro)。是不是可能因为这个对心脏有很不好的副作用导致严重失眠突然出现呢（
如下所述，吃的当天晚上两次心动过速，严重失眠也就此开始）？ |

如果你妈本来就有心跳过速那是可以的，metoprolol是walmart的4dollor |
