acute? How is the blood flow in vertebral arteries?
My impression is that the syncope was due to brain stem infarct rather than
SSS. SSS is probably an incidental finding.Having said that I haven't seen a
case with retrograde basilar flow from SSS(just realize our hosp doppler
ultrasound never reports basilar :( ）

suggestive of sub capsular bleeds, presumably at bleeds with re-bleeds. If
the patient does not have trauma, then coagulopathy with meds or neoplasms
should be considered. I don't see the typical wedge-shaped hypo attenuation
seen with arterial infarct, But venous infarct can't be excluded. Although
post-infarct hemorrhage is not very commonly seen.

just from the symptoms, it is likely acute myocardial infarction (AMI). it
could also be severe angina without infarction. in the majority of cases,
the primary cause is vascular. alternatively, in certain circumstances,
problems in the conduction system can cause hemodynamic disturbances severe
enough to cause heart attacks.
AMI, depending on where it occurs, can damage the conduction system too.

transformation based on the initial presentation.
So elevated BP is from the stroke not the other way around.
The workup should include a MRI/MRA to establish a diagnosis of acute
infarct and rule out anurysm/AVM, respectively.
Treatement would be to moderately lower the BP, like 140/90. Too low would
worsen the stroke, whereas too high would make bleeding more.

I guess this is a brain stem infarct or capsular infarct which is usually
very tiny but causes severe symptoms. Fortunately these stroke symptoms
usually imrove even though very slow. From your story, I don't think there
was any theraputic window to treat this stroke (> 3 hours). Therefore, an
earlier diagnosis may not change the outcome.
Nevertheless, I do think they should not misdiagnose her at the first place. |

Khat is so popular in Yemen, its cultivation consumes much of the country's
agricultural resources. An estimated 40% of the country's water supply goes
