distopia, comparing to those of Arthur C. Clarke and Asimov. He is obsessed
with totalitarian, machine vs human, drugs, and memory, reality, etc.
I read an interview of him. During the 50's, he was the target of the FBI
for possible subversive proclivity and drug abuse. His _The World Jones
Made_ gave me such a great impression that I totally understand why his
worlds is so depressed.

No Union to support them;
No rights to strike;

Did the original group set up side by side control saying that the small

时。然后round到晚上8点，attending让我走了，自己继续把没round完的病人看完。回
家接着做了3个小时的dictation.
呼吸科的问诊是有套路的。问两次后就成了熟练工种了。查体也很快。慢的是分析影像
资料和写notes。治疗上来说，对COPD/asthma，我们有一个cocktail：albuterol,
spiriva, pulmicort, theophylline, perforomist。对肺炎，就是分清CAP和HAP。不
知道处理的在notes里一律写上：will d/w attending about plan。没整对是因为不知
道怎么微调那些cocktail。
consult病人比floor的病人容易处理多了，只focus一个小块。但熟练以后，floor病人
也很快能看完。我今早上从6：45到10：45共看了8个floor老病人，一个ICU老病人，办
了4个discharge，还吃了一顿早餐。我们这儿看一个病人是指notes写好，order写好，

住院医和学生的本质区别是上手而不是学书本知识。一定要自己亲手管病人，自己定夺
一切诊疗。
我当时是第三个月转呼吸科。第一天管了17个病人，其中7个新收的。早上9点到，晚上
8点走。Attending见我把17个病人全搞定，激动得跟我说：明年有空去我私人门诊转一
个月。虽然当时每一个病人几乎都没整对，但一个月后出科时，对呼吸的东东还是信心
满满的。

不过这个mm，spreadking说得还是有道理的，不能上头nice你也心情放松了。不自己管
病人下order，是很难真正学进去的，怎么都是一个旁观者，我觉得从你描述的来看，
你还停留在observe的阶段，而没有进入intern的角色。你现在不是OB,不是
medicalstudent了，而是真正的住院医生了。第一天第二天也许可以，但是一个月都这
样肯定不行。你可以主动要求去看新病人，然后从头到尾管这个病人，让护士有问题就

UW 41 offline cases: 我当时和partner每个case仔细做一遍，虽然不像online有反馈
，但对练临床思维，补充protocol非常有帮助。最后考试与41 case有异曲同工之处，
遇到复杂case也不慌神。

有个好的filter，水保持流动就有足够的氧溶在水里，除非气压低，气泵可有可无。想
种水草要买光谱适合水草的灯管，2根以上比较好。可以先种点水榕（anubias),爪哇蕨
（java fern)。垃圾鱼可以用common plecostomus,这个便宜，好养，弄条放缸里。
鱼：(malawi慈鲷颜色鲜艳，小鱼比坦湖的便宜些，好养点，养非洲慈鲷可以从马拉维
慈鲷开始）
