to have a small frontal fontanel. Now it is 0.5x0.6 cm at her 3.7 month age.
They are now worried that her frontal fontanel is closing too early that it
may limit her brain development. The doctor in China suggests to do an MRI
scan, and they are hesitating to do this for her at this stage. Anyone knows
anything about how such situation is usually handled in the US? Here are

我家那个儿医在check up的时候会跟我们说娃的头骨还没长好，用手摸摸，然后让我们
摸摸感觉一下……
肚子倒是按过，而且娃超级不配合，直接打挺，儿医说摸着肚子硬，不知道是肝比较大
还是那块有肌肉，就refer了一个B超给娃做，结果出来也算正常。
“假如前囟的部位比头颅表面突出来，象个小鼓包似的，用手按一按，感觉很硬，绷得
很紧，这说明头颅里面压力增高。引起压力增高的原因很多，最常见的原因是感染，如
各种脑炎、脑膜炎、颅内有出血或脑肿瘤、脑积水时也都可以引起颅内压增高。有时小
儿吃鱼肝油过多，造成维生素A中毒也可引起。”希望这个儿医这样按压宝宝的卤门只
是为了检查宝宝的健康状况。

发帖数: 383 | 7 唉，我家娃也是，所以为了这个肋骨外翻我google了半天，结果发现按中文网育儿简直
是害死人啊。
首先，貌似国外没有肋骨外翻这一概念，根本查不出“肋骨外翻”对应的英文。
其次，俺在baidu百科“肋骨”一词上看到这么一个说法，“1、如果你发育未完成的话
：孩子的肋骨外翻是由于大多数孩子的肚子都较大，加之肋骨偏软，张力的关系，久之
就会出现肋骨外翻的症状。随着孩子的生长，胸廓展开后症状即会消失。微量原素在正
常水平的低线，应从饮食上继续补充为宜，并多到户外活动，接受阳光的照射，促进钙
的吸收。” 俺觉得这话比较靠谱。
另外，前几天我带娃checkup的时候问过儿医，他说如果缺钙，肋骨的表现应该是可以
在肋骨上发现小的bump，单纯肋骨明显不代表缺钙。
附我查到的缺钙表现：

比如：3.Teething delay, uneven teeth, upper and lower teeth not sew,
malocclusion,
loose teeth, easy to fold collapse, premature tooth loss.
我姐姐的孩子4个多月就开始长牙了，够早吧
4. Psychological disturbances, are not interested in the surrounding
environment, as in the past and lively.

5.Delayed fontanel closure, often still closed after 1 year and a half.
没delay都正常。
我觉得大家还是小心为妙，我姐姐的娃补的这么多，还有点鸡胸那，要是不补很难想象
的。

发帖数: 1239 | 14 这两种swaddle我都用过，更喜欢HALO的。summer infant那个腿部空间不够大，我家娃
50 percentile，算正常大小，腿在里面都不能保持natural position，最后还是退了。
去医院的包裹包括软包硬箱我打了3、4个，有生产包，产后护理包，出院包，最后用上
的不多。因为生的快，生产包基本都没用上。说几个我觉得有用的吧：
-- Depend护垫。医院的太难用，还会漏，这个depend很舒服。
-- 如果准备母乳的话，带上nursing bra，和feeding pillow，不然肩膀、手腕会很累。
-- 洗漱用品、浴巾
-- 小人的手套
-- 自己出院的衣服（建议带上围巾和帽子）。我自己就没注意，结果出院回家就15分
钟的路，就被车上空调吹病了，到现在两周了还头疼。lg怕是pe-eclampsia，出院后第
二天又把我带回去了，在医院耗了4、5个小时才给放回来。
-- 小娃出院衣服长袖onesie就够了，怕腿冷的话就薄、中、厚的小毯子各带一条，到
时候腿上一盖就可以了。

An 8month old infant is brought to the ED due to vomitting and decreased
urine output. 3days ago, he had a fever, sore throat and ear pain. he was dx
with otitis media and rx with oral amoxixillin. today his temp is 40C, PR

这两天把材料递上去以后，开始诚惶诚恐中。一直混买地，一遍遍读别人的考经卖经。
早就想cs过了我也写个考经。
背景 ，yog>10，step1和ck都是白菜分。准备时间不足1个半月，全职上班。每天学习3
-4小时吧，没有请假复习。英文中等，平常交流没有问题，就是一着急就容易时态不分
，单复数不分，男女不分。最近在做CRA的工作，所以和patient讲话这一块儿每天练习
机会很多。
4/18号考完ck，走出考场的时候感觉horrible，想着还用得着准备cs吗。无奈cs老早已
经定在6月初了，那会儿想改时间已经几乎没可能了。没有时间郁闷和痛苦，考完之后
立刻上网发帖找partner，tutor。
大概考完ck的第4天开始，我还一页书都没看过，什么都不知道，就跟一个考友练了两
个fa上的大case，真的特别感谢她没有嫌弃我。但我觉得我太弱了，不能拖她后腿。就
又找了个学友，当然这个学友也是比我强的，她告诉了我很多protocol，包括问诊体检
什么的。这样我们一起把fa的大case通过skype练了一遍。我练fa大case时基本没有提

发帖数: 117 | 23 就是怕抽不出来丢人和怎么善后。他是很细心的人。之所以有心理issue 也是去观察的
时候亲眼见一个医生抽不出来，长针已经进入病人的l4-l5，只有一滴四不像的液体流
出来，最后让助理挤压弯曲病人fontanelle 和头颈，才出来3。4 毫升，（病人是早产儿
）。自此就有了心里issue.
是不是有机会自己亲手练习会好些。目前那副场面是很难忘记的。
反正做peds 是压力很大的。成年人的是不是好做些呢。
