发帖数: 18 | 1 新年快到了，这两个星期陆续听到好几个朋友的好消息，都是毕业超过20年的CMG，基本上都拿到了比较理想的地方。
我原来说过要写一个老毕业生宝典，考虑了很久，因为许多东西涉及到很多朋友的个人隐私，而买买提这个网站似乎有爆料的传统，所以只好放弃这个主题了。不过有两点我还是想写出来给老CMG一些关键信息。
对于毕业超过15年的CMG，根据我了解的情况，大致来说有两个因素是必不可少的：
第一、行内人士的推荐：推荐人的级别，基本上也就决定了你能进什么级别的program。关于推荐信，去年鞋匠同学有一个很不错的帖子，请你去考古（如果你找不到，那是因为你心不诚）。
第二、临床经验：国内多年的临床经验会有很大的帮助，但是美国对应专业的临床经验才是关键，而你在得到美国经验的同时，至少一到两个推荐信大概也就基本上有指望了。
实话实说吧，如果没有前面两个条件，毕业20年以上的你基本上可以放弃USMLE这条路了。但是，条件是人创造出来的，你下决心走这条路，就要下决心去创造这个条件。有人说我都毕业20年了，再去做externship又要熬一年，岂不是毕业时间更长了？可是，毕业20年跟毕业22年有实质区别吗？没有。 |

原因有三：
1。CS第三次pass，出结果也要十月底了，拿到certificate要十一月了，估计拿到的IV
应该是比较少了，花那么多申请费，有点浪费。还有已经申请了一年的记录。
2。再利用一年的机会，多做OB，多做externship，多跟PD，椅子男套磁，大搞
connection，build up strong package，来年一举搞定，这叫磨刀不误砍柴工。
3。有利于放松心情，消除烦躁心理，一举攻克CS，了却一块心病。

Clerkship - our third/forth year clinical rotations. Under physicians' supervision
we can have some hands-on clinical experiences.
Elective - this is relative to the "required" or "core" rotations. In the
third year our core rotations are IM, Peds, OB/GYN, surgery, Psych, and
primary care. Our available electives are Neuro, anesthesiology and radiology.
In the forth year, we can select pretty much any specialties, such as Orth,
plastic, nuclear medicine, Rad Onc, Derm |

了, 我只能看, 不能动手. 而且她干活很麻利,实在也找不出我可以表现, 拍马屁的地
方.没两天,又提出, 因为我不能动手, 她也不好写什么,只能说我跟她了解了一些医疗
系统.
太郁闷了, 我现在只有这一个OB. 如果找, 也只有可能找一个中国医生了,听说中国医
生的推荐信, 人家会打折扣. 而且,我老板是中国人. 不想再要中国人的推荐信了.
大家出个注意吧, 我还在准备CK.是把CK放下,去外地搞一个OB 或externship做呢, 还
是首先把CK赶在8月底完成, 就凑和推荐信了. |

just shadowing/following an attending and no direct patient contact. You
may attend some case discussions, meetings and lectures.
An externship on the other hand, offers some hands-on U.S. clinical
experiences. Students/graduates can touch/examine most patients. Externs are
like interns but without a contract with the hospital and their work is
unpaid like students.

做神外，泌尿的什么的都有。
现在中国人做医生的多了。每个大医学中心都有一大帮中国大夫。这是很大的校友资源
和同乡资源，可是我就很少看到有人利用的。说到底就是面子，可以在互联网上蒙着面
寻求一言片语的回答，但不肯在现实中拉下脸来百般钻营的找个MENTOR指导。 |

prematch offer，决定从了。共收到 5 个 IVs so far， 这是第一个 interview 和
offer。在 interview 时，就我一个老中，其余全是老美，老印。PD 也是老印，不过
人很 nice。看来今年实在是形势严峻，我估计大部分 programs 被老印，老巴给
flooded 了。我们老中还需努力。

原来是打算申请Neurology 和 Pathology的, 因为Neurology 需要申请Prelim, 所以就
计划去做3个月的IM externship. 病理做了两个OB. 因为研究背景是Neuroscience, 所
以只做了个很短的Neurology OB. 考虑很久到底儿该选什么科, 做完OB 到申请的时候
也没太想好. 应该是更倾向于病理吧.后来看了看usmle forum 上面病理的thread,觉得
比去年,前年竞争强了很多, 怎么也要再选一个做backup吧, Neuro要申Prelim 和
Advanced, 再加上病理3个,太麻烦了, 而且神经科本身挺深奥难学的, 自己也没有任何
神经科的临床经验, 就索性申请了内科. 内科的优点是时间短, 就3年, 工作机会比较
多, 因为老公的工作性质, 以后要找Faculty, 工作机会比较少, 也是在迁就他吧, 两
个人在一起很重要, 两个人都开心也很重要, 我希望他能继续做他自己喜欢做的事. 再
有内科是医学的基础, 以后也可以照顾父母, 公公,婆婆的身体健康. 我的研究背景对 |

鞭策。最近忙着房子上市，externship, observation, step3 的事情，心情有点急躁
，把小孩也吓着了，也墙也不敢碰，怕搞脏了连累到房子卖不出去，领导大人也因此严
厉批评了。回临床不单是为了经济上富足的原因，也是为了一点人生的追求，希望能够
因此而完整一点。只要这一目标得以实现，其他的一切都无所谓。想来自己也算有房子
有地的小富农了，这回上临床，光卖房子就得把全家这几年的积蓄给亏个一半，现在的
行情是有房无市，房子越大越难卖，更别说上临床做牛做马从头来过了，一家老少还要
跟着挤apartment。但是为了一个做医生的目标，为了让领导大人能够stay at home -
相夫教子是她的目标：-)， 为了让小孩有个更大的playground，我们全家都从了! |

You can do the google. There are several hospitals offering the externship
or mini residency, however some of them requiring the ECFMG certificate when
you apply. Most likely, you need to relocate.

院FM做的OB并做了笔记。第二个面试PD问了我的OB和EXTERNSHIP，并做了COMMENTS。当
然我们花了更多的时间谈当地的教育和生活。
面试前我和RECOMMENDERS都打了招呼，他们在LOR里都留了电话和EMAIL。象刀老师得
answer电话backup的，either the program is very, very competitive, or the
candidate didn't impress the program very very much on the scene - 当然多数

求attending帮忙，只能随着他们的schedule，绝大部分时间是weekday实验要提前安排
好，不要耽误进度。我当时就是每个星期去两个上午。平时做实验。
2.如何和老板说？
看你老板的为人和你的关系，我个人的经历告诉我老板永远就是老板，出发点就是不一
样。我就是到了考试都考完了，要推荐信的时候才告诉她的，之后也证明了我以前的判
断。
3.如何找OB？
发动周围所有的资源，agressive一点，主动去联系。先不要太挑剔，只要你做了第一个
OB，认识了第一个attending，通过他就像一张网，你会认识更多的attending，connec
tion是这样来的。当然，万事开头难，开始要好好准备。
4.full time or part time?
