. 最近这段时间厉害了, 两个多星期前腹部涨痛特别频繁, 原来医生给的Hyoscyamine
tablet吃了好些 (doctor think i have irritable bowel syndrome), 但是第二天,
尤其是下午4,5点又开始了腹部涨痛. 揉肚子感觉很多气, 可是出不来. 现在吃晚饭都
有压力了, 吃了后感觉更厉害, 侧身休息会缓解.
两星期前做colonoscopy, 当时医生说所有结果都是正常. 昨天拿到biopsy result.

insurance有1200得deductible，超出得部分20％
in network得保险公司有折扣价。以下都是折扣以后得价格。
gastroenotologist fee $251
facility $780
麻醉用药 $500
lab $500
疯了，以上都是insurance之后得部分，超过了$1200得deductible，最后自己要交660
左右。 今天又接到麻醉用药 $500得账单，这个居然是out of network，20％不能用。
这样加起来这个endoscopy就要自己掏$1100。 这个还能跟医院argue么？？？ |

bowel endoscopy.两个procedure,于是账单也就比预期高了不少。不知道这种情况正常
吗？医生现在说当时觉得有必要，我们当然也没有足够专业知识加以判断，所以来这里
请教。当时也没发现ulcer,只是有点redness，虽然后来h pylori是阳性的。谢谢 |

It is possible that a biopsy was taken during the first endoscopy, a clip
was put in to stop bleeding.
It is very unlikely to leave a clip during endoscopy unless it is put in on
purpose. |

说还比较挣钱的工作。in pt, floor的工作不适合你。其实俺老婆三年前护士BSN毕业
的时候就只面试了四份工作（美容减肥诊所，out pt endoscopy center,一个
assistant living place招护士就是发发药之类的，还有个part time药店里打流感疫
苗）。半年前还面试了一个school nurse的工作。这些都是周一到周五白班的工作。最
后她去了endoscopy center,周一到周五，7am to 3pm的工作，不过基本上1pm就结束回
家了。现在有孩子了，又在上FNP program, endoscopy unit的工作已经改成Per diem
了，现在平均每周工作一次大概6小时吧。她的curriculum俺也打听过了，俺也和俺

First, do a blood test including a liver function test.
Second, do an ultrasound of the gall bladder and the liver if nothing is
found in Step 1.
Third, do an X-ray of the chest including the esophagus if nothing is found
in Step 2.
Finally, do a gastric endoscopy if nothing is found in Step 3.

the symptoms 2-3 are very similar to my problems a couple years ago. I went
to see the doctor and got some prescriptions, and the doctor said I did not
need to do an endoscopy immediately, until if the medicines didn't help. But
the medicines turned out to be helping a lot so I never did the endoscopy.
Now my stomach almost does not have any problem.

Better post the results online, then somebody may help you. Otherwise,
difficult to tell what you mean "乳头状瘤" and "脓性息肉". To my knowledge,
those are not routine descrition on endoscopy report. |

有癌细胞，说是幽门杆菌引起的胃溃疡。
上个礼拜在加拿大复查 ct没有问题，但是昨天切片结果说有癌细胞。专科医生说可能
是贲门癌。但说不清楚是哪个阶段，要到做手术时才知道。
现在有几个问题。
1. 怎么两次切片会结果截然不一样? 能否推断就算不是false positive 也多半是早期
～～几次ct endoscopy 都没有发现明显的肿瘤啊。我爸胃口很好 没有吞咽困难 没有
体重减轻，现在还一天爬两次山。
2. 为何第二次切片结果不能确定是何种癌细胞和那个阶段？一定要等到手术看周围淋
巴情况有没有转移？听说国内切片结果结合ct就可以确诊？
3. 加拿大的白痴医疗制度。排队可能要等很久，而且医生临床经验不够。我国内有个
铁哥们在大医院，上次有个同学胃癌从诊断到开刀才花了两个星期。我是不是应该陪我
爸回国复诊开刀？

trying to figure out whether it is bleeding caused by benign gastric ulcer
or bleeding gastric tumor. Endoscopy still is the best way to go. What's
more, you need biopsy to make the final diagnosis no matter what. Endoscopy
is only way to get it unless you will have a surgery. PET/CT really is not
that sensitive nor specific for small GI tumor. If it is very positive, of

in vivo optical imaging). And we are talking about 前景. I would say image-
guided-surgery is a emerging field with great potential. I am not surprised
few people are familiar with this. Not many people even heard about this. If
you talk about basic research, current technology, no doubt microscopy is a
big area.
说反了吧，最大的市场显然照相和显微镜技术，其次是endoscopy，都是超过几十亿的
市场。再其次好像是OCT和光谱学分析，别的真是想不出什么有市场的应用了，什么荧

东西很快，被一高年资菲
律宾护士嫉妒了，老婆是唯一full time RN, 接受培训成为head nurse,昨天
administrator让我老婆去问那个菲律宾护
士询问一些关于endoscopy nurse的书和材料，因为那个菲律宾nurse以前是一个医院的
endoscopy的head nurse,
没想到此bitch竟然说没有这方面材料,都是在med/surg经验上积累起来的（言下之意，
我老婆没有工作经验),我老
婆很厉害，立马反驳抵挡回去，还外加很微笑地和administrator complain了那个
bitch一把。其实老婆去培训第一
天，就有好多老美职员私下里和我老婆说过那个bitch难处。第一天那些美国同事就把
那里的种种小道消息，人
跟人的关系就都告诉我老婆了。而且我们医院的一个nursing supervisor也在那个
bitch全职工作的医院做nursing

病人转换很快，估计每天要看很多病人
可是根据自己去做endoscopy的经历，觉得那里的护士就是intake 病人，收集基本信息
，签consent form，上piv，然后等轮到的时候，tech和护士推进去，然后是另一批人
了，再后面就是醒了评估下，没事的话就放病人走，有事再多观察会儿。
那个school nurse还有点我忘了说，还有些teaching project要定期做。小小孩的还有
针对父母的teaching。 比如我在那儿混的时候，一个是交洗手（针对小孩的），一个
是营养方面的和lead level（针对父母的）。后面这个还有involve一个school
distric的dietitian，当然school nurse必须coordinate整个project。 |

Majority consistent to GI loss (malnutrition/malabsorption)
Edema is due to hypoalbumin and low protein. Originally lower extremity edema was thought to be due to systemic involvement. But when I saw "4+ pitting edema up to low back" on PE on day 2, I knew it could be due to low protein level. I appreciate the good PE you did. :)
Hypokalemia is most likely due to GI loss too.

美国的保险，很多也是不报。医疗保险的条款，3种类型：1，试验性experimental，
2调查性investigational；3，未证明的unproven，这三种类型，保险公那都不给报
尤其是第三种，unproven,里面的悬念就很大。
我不是医学专业的，白蛋白是不是unproven，我不知道。但肯定有别的unproven。
另外，我们同事，去年做过一次小手术，都没住院的那种，也是unproven，一样不给报
同事不爽，去appeal，结果你猜保险公司说什么？
保险公司说：你找你们自己公司的HR去appeal
另外，保险还有各种各类的exclusion。就比如近视眼手术，肯定被exclude掉了
另外，保险公司在报销药类上面，能报销的幅度也各有差异
比如，保险公司的条款上明明写着“prescription自付比例10%”
一种很常见的做endoscopy的药，moviprep，自费的比例超过50%
可见保险公司的花样非常非常多
再比如，“你保险不好，很多药医生都不给你开”。这也说明你那个医生的水平好。
我们这边，医生、药、lab分开。很多检查，明明是不必要的，医生也给开了。

昨天，这些人折腾了一天，白天，晚上皮啦啦，也不知道谁把谁杀了，谁把谁带走了。
反正我先生和孩子被绑架了，就不知道是不是已经遇害了。我本来是打算睡觉的，但他
们头上，地下装电磁波攻击器，让我几乎窒息。虽然我没倒下，但其实他们在我身上扎
的毒针已经蔓延，我能感觉到我的呼吸有窒息的感觉。我想我对他们已经非常仁慈了，
但他们不依不饶地攻击我，欲置我于死地，让我非常生气，特别是肆意伤害我家里的人
，绑架我先生和孩子，而且有许许多多无辜的生命毁于他们之手，有必要把他们曝光。
当然我还没分析这里边是那几个人具体攻击我，伤害我的，但他们显然都是一伙的。鉴
于他们伤害我的头部，胸部 和胃部，我就把这几部分的医生都曝光，受害者慢慢去调
查，追查他们吧。

如果最终是对的话，我觉得是近50年来数学的重大结果
可能没有FLT对数学的促进大，但是不比费尔马大定里的影响小
对搞数学的来说 证明相差为70000000的素数有无穷多对和证明相差为2的素数有无穷多
对。并没有实质性的差别。意义是一样的。

You may also have lactose-intolerance. It's common among Chinese people.

of treatment, sounds like you have dyspepsia, if you worry about enough,
maybe go see your doc and get an endoscopy study |

depends on your doctor, it's not a good thing to do but, let's say someone
comes in to the office complaining of abdominal discomfort, stated he lost
about 20 lbs in last 1 month, and now is having trouble swallowing food,
plus he also has strong family history of cancers, i don't know, maybe in
stomach or esophagus, with that presentation, it's pretty hard for the
doctor not to consider endoscopy,

could be, you can try some over the counter prilosec or pepcid or even
zantac, if not much better after a couple weeks you need to see your doctor
to get some lab works and ultrasound and maybe even endoscopy |

if you're a female, probably also need to look into ovarian etiology, always
thought pelvic exam is part of work-up for female with abdominal pain, my
pelvic exam is CT of abdomen and pelvis,
other test i could think of is endoscopy,
if HIDA scan with cck injection is still normal, probably is not gallbladder
related, but you could always go to extreme and get an exploratory
laparotomy, |
