得那样。
顺便说一下，院内感染，除非是HIV，hcv这类血源性感染，否则基本只能算
complications，就是并发症，是不能起诉医院的。接触性感染，contact isolation医
院太多了，要是这也能起诉医院的话，所有医院都要关门了。每天在医院里感染新细菌
病毒的人太多了，这是不能完全避免了的。比如术后各种感染，cdiff，医院获得性肺
炎，catheter导致尿路感染，美国即使诉棍横行，在医院干这么久，也没听说有因为感
染起诉医院的case。就像前面有位id说得，你在医院得了流感要不要起诉医院？ |

首先正常的epidural是打在脊柱硬膜(dura)外， 所以才叫epidural. 不是打在CSF里。
但临床上硬膜穿刺常有发生。 出现后怎么办呢？ 传统的办法是放弃。 换一个椎间再
做。现在比较流行的是直接把导管放在csf里， 这个叫做subdural placement of
epidural catheter. 是一个补救措施，并不是正常的做法。
不管硬膜外或硬膜内放置， 麻醉上了以后孕妇血压都有一定程度的降低。 会影响到胎
儿的供血。 如果胎儿耐受不好， 胎心会降低的。 胎心减低有多种可能的因素，
oxytocin过量也会。但比较少见。 |

10年前，我们大院有个小伙子医学院毕业，找到广州一家大型省级医院的工作，结果，
因为是乙肝病毒携带者，居然被医院拒绝了！惨啊。
那么，我做点好事情吧 --- 美国医院并不拒绝乙肝病毒携带者。
我不是开玩笑，这是真的。

tissue, or cryoabaltion, the probe that emeits cold or whatever to freeze
the tissue; either case you have to place the probe directly into the tumor,

even in chemo-embolization, like they do for liver mets, you still have to

进入大动脉,射入能显影的东西.
手术前后的准备和观察阶段是很重要的.因为手术后要平躺至少24小时让大动脉那儿伤
口愈合,否则很容易大动脉出血!很危险! 因为这个原因,手术后不能大小便,所以建议手
术前一段时间别吃别喝.也因为这个原因,一般给男病人在手术前做尿道导尿术,就是把
一个橡胶管子从阴茎口插入到里面,这样膀胱里的液体能自动通过导尿管排出,不需要站
起来小便.这个插入的过程是很痛苦的,试过就知道了.
不知道这个冠状动脉造影是从哪个动脉进去,反正从股动脉进去的话,手术后要平躺让动
脉伤口愈合. |

要反复x-ray，这个辐射很大。在中国，未婚未育的不能进介入组，由于计划生育所以
只要生过1个孩子then开始具备入组条件。辐射量？病人1次/life，医生1-3次/day，所
以要防护，而且衣服内有计数器，每次做必须佩戴，如果计数器到达limit，然后今年
就over, 转到读片室写报告去，中美都这样，当然你非要休假那没人说什么。 |

我們一直希望能將人為的藥物及儀器的使用降到最低，像是induction用的 Pitocin,
插上後就不能自由行動的internal monitor, catheter等等【Epidural除外！】；雖然
我個子小，但我跟老公都覺得我身體很健康，自然產一定沒問題，何況我媽也是這樣把
我生了出來【我媽比我高一些些；我出生是2500g，兩個妹妹都有3000多】。
在11/5清早發現落紅的時候，我們還想，這個小冰箱真準時，就選預產期這天來報到。
我的落紅一點也不痛，宮縮也不疼，我們一點都沒想到會有這麼多問題。到了醫院之後
發生的點點滴滴，一件件都在我們的預料之外，徹徹底底是計畫趕不上變化。早早就上
了Pitcoin、被人工破水、插導尿管、internal monitor，生完後還用了一堆麻藥跟止
痛藥，完全就是一個極度不自然的birth story.
說沒有遺憾是騙人的。或許是我對於自然產有太多的期待，總想像著寶寶出生後立即被
抱到我身邊依偎著，我很滿足流淚的那一刻。

撑到满41周的OB，42周的话，母体内的环境可能不会比母体外好。在我所有知道的双宝
中，有两个妈妈是40周零5天，那是最长的了，我结果也是40周零4天发动，第二天生的
，也算破了个记录了，呵呵。
让LABOR自然发动有很多好处，如果想胸喂的话，尤其重要。不论双胞胎还是单个，满
38周才是真正的FULL TERM。如果孕期没有任何问题的话，BED REST只有不好（LOSS OF
BONE DENSITY），经常运动有助于顺产，并且保证不长太多的份量，以至于到最后几
个星期无法忍受。
可惜的是，一系列的MEDICAL INTERVENTION造成了非常困难的LABOR，我到医院的时候
已经开了6指，OB给我挂水，但是拒绝了我用CATHETER的要求，而且他们怀疑我漏羊水
，给了我三种抗生素，我拒接用PITOCIN和EPIDURAL，labor总共23小时开10指，
TRANSITION有4～5个小时，结果在PUSH前导尿，放出1500CC,把护士吓一跳，难怪BABY
A下不下来，有那么大的bladder挡 |

We does not know if he has reflux or not.
Personally, I don't even like the idea of catheter insertion. But we have to
do that to get a clean sample.
照影计was a bigger concern.
And after the VCUG, the only useful result that can come out is to put him
on low dose antibiotics, which we really don't like. So we decided that even
if he had level 4, we don't want to put him on antibiotics. We will just

clean soon (next day, or so), it is may just a light damage, if there is
urine retention ( urine can not pass out), it needs immediate medical
attention. In general, small damage will not be any harm for long run, for
sometimes due to scar tissue, that can cause the narrowing of urethra, you
need to consult a urologist to make sure that will not happen, and check

problem sounds like the catheter was looped inside the bladder and advance
into it resulting a knot. When baby''s urethra is so narrow and forcing
pulling out the knot, trauma can be caused at bladder neck. Baby's prostate
glands are very small, hopefully not injury done there.
There is no balloon with direct catheterization, so it should not be the
error not to deflate the balloon.

experienced nurse soon... It IS very hard to put a intravenous catheter on a
moving child, it's definitely necessary to hold them tight and still.. and
to some extend, for some chubby babies or adults, even the best nurse can't
really find a superficial vein ( IV on breast or forehead happens,
squeezing blood from heels is common in children if blood is needed urgently

一下我爱宝宝版，版上的妈妈们，准妈妈们，所有的人都很nice, 在这里也学到了很
多东西，得到了不少的建议和信息。
我的预产期是5月3号，可是从37周产检开一指之后就再也没有任何进展，39周的时候我
的ob帮我做了membranes stripping, 希望帮我加速开指。结果那天下午回到家确实有
一点点见红，当天晚上以为要生了，兴冲冲跑去医院，结果还是1指，灰溜溜的又回来
了。
5月 1号：产前最后一次产检，还是一指，50% dilation, position -2。医生笑着跟我
说”you have a very stubborn cervix”。最后决定5月3号去催产（之前一直纠结要
不要催，最后还是妥协了），还帮我约了第二天去放气球 （Foley balloon catheter
induction of labor）
5月2号：1:30pm 出发去种气球。3pm 种完气球，刚从医院出来肚子就开始阵痛了，痛

有的血管都试了，插进去，在里面搅一阵，偶尔摸到血管，然后又滑掉了。就没一个成
功的。我实在忍不住，去跟医生说，能不能不吊了。
娃实在太太太可怜了，现在不光她有心理阴影，我也有了。

epidural catheter从硬膜外腔出来了。当初放的时候留得不够长，固定得也不牢，你
在床上又来回动所以出来了。

经历，给各位新手妈妈一个参考，美帝治疗这方面确实挺麻烦的，千万不要重蹈我的覆
辙。
说来自从我家宝出生一直就一波三折，先是生下来之后一小时喂他奶他差点窒息被CPR
，又是体检查出锁骨骨折，然后月子里脐带感染在儿童医院住了5天挂了无数抗生素—
— 终于熬到第六周结束，我还以为平静的生活即将开始了，结果第七周开始就乳腺炎
发作直到现在第十周了还没痊愈，桑心啊。先来描述一下前因后果吧。
宝出生后一直是大部分泵奶，也说不上为啥，可能是被月嫂影响说这样月子做的好 （
其实是错误的）。导致基础就木有打好，因为泵奶不容易把乳管弄的很通顺，总没有宝
宝吸来的有效。
第七周：
周一开始发现左胸靠近RT的部分有一块硬的，按压还有一点疼，心里知道多半是乳管阻
塞，就想着白天让小宝多吸吸，自己多按摩按摩。其实月子里堵塞的情况也发生过好几
次，都是热毛巾按着硬推推开的，痛到掉眼泪，心想这次也差不多吧。结果白天小宝一

老公在1个半小时车程之外读书。我做生物博后。
小朋友现在18个月，刚开始上daycare。我婆婆下个月就走了，10月份开始我一个人带
小朋友，所幸DAYCARE很近（也谢谢之前版上大家对DAYCARE选择的建议）。老公每周回
来。
想要老二的动机是：
我从小到大晕过几次。最近终于下定决心彻查看了cardiologist.第一次visit作了EKG
，cardiologist跟我说，结合症状及EKG，我可能有一个rare syndrome.回家自己做功
课，发现这个syndrome随时随地都能死。很depress。一方面觉得小朋友可能以后就没
娘，最重要觉得是爸妈以后就没有儿女了。
那时候就再想，要不要再生一个，这样小朋友有个伴，第二个就要跟我姓，我爸妈也能
有所寄托。
后来，做了cardiac catheter的电生理检查，医生说all negative.我就觉得我是如获
新生一般，不用死了。能看小朋友长大，也不会白发人送黑发了。
现在，这个要老二的想法还在。乘自己还没到35（现年31），乘爸妈还在（我爸得了
