设的讲座，本来还是挺反感的，因为本来就值班晚上没睡好，还要牺牲早上睡懒觉的时
间去听讲座。但是开始讲了以后，还是受益蛮大的。这只是第一堂，后面还有follow
up session，一直到六月份。这个老头还是满好玩的，到处做这种讲座，反响很好，参
加的医生课前课后的表现都有不同程度的进步。
我所说的都是以门诊为主的。但是一些细节还是共通的。其实大道理我们都知道，但是
细节方面，还是有很多改进的，特别是我们老中，在以前上医学院的时候从来不注重这
些。美国花很多精力在这种医患心理学上，原动力是避免医患纠纷，提高医生效率和病
人满意率。对新开始的住院医生来说，这是和我们以前学习的非常不同的环境。
在医生走入病人房间的那一刹那，病人和医生的关系是不对称的，所以潜意识中病人有
一种紧张和tension，如果医生在表情和肢体语言中流露出什么负面的东西，哪怕完全
和病人没有关系，病人也会感受到并且加深这种不平等的感觉。要和病人建立信任，医
生从一开始就应该意识到这一点，而从细微之处改善这种紧张的感觉。

Case#1. Cl for depression, as pt withdrawal, not willing to verbally
communicate with primary team. So I went to see pt, found he's pleasant and
talking fine, but noticed him spit frequently. Then I asked "your doc is
concerned you don't talk that much, is there anything bother you that makes
you not willing to talk?" pt replied "doc, you see I have so much saliva,

时就有，但是可能症状比较轻， 没有治疗。最近几年变得bothersome，基本上每一两
分钟都会咳嗽一下。看过几个家庭医生，一个ENT，都是按照某种可能性（比如过敏，
Post-nasal drip）开药试一下，不行再换一种；其中一个庸医发现我TB阳性，就不管
咳嗽开始治TB阳性了（我的理解是阳性可能是小时候种过卡介苗的原因,一但测出阳性
就总是永远阳性不用再测了，以前上学和绿卡都测试并治疗过）。
你的症状1,2,3和我完完全全一样。干咳没有痰，睡觉和跑步时绝对不会咳嗽。第4点可
能也有，我偶尔也会感觉到喉咙突然很痒，但是不觉得和身体姿势有关。还有一个重要
的症状是我有很严重的canker sore，嘴唇很容易破点皮就发展成一个很大的溃疡，一
般都要几个星期才好。这种情况一年会有好几次。如果连续几天晚上熬夜的话出现
canker sore的可能性很大。还有一个症状就是早上起来嘴里总是有点苦涩的味道，但
是没有泛酸和烧心的感觉。

kindergarten，上daycare，就要在daycare多待一年。这点上，如果是男孩，可能就再

年，不需要纠结了，可是女孩，我好像记得小菜妈说过，数学一定不要开始太晚，免得
错过敏感期
3，各种课外活动多，也有校外来开音乐舞蹈啊之类的课，如果喜欢去，可以少占用周
末时间

average的。我就当他小，算了。

他作业，我没法不陪的，先要读书呢。
