Step3考场壮烈回来。第一天挺累人，最后一个block甚至做到有心无力状，看着题反应
不过来，导致来不及做完。非常没有信心能否通过。所以match之前先考Step3的童鞋，

复习资料：
UW 52 online cases : 基础。最好复习早起找高手一起过一遍，尽快摸熟软件，进入
状态。第二遍找一位水平相近童鞋过一遍，不断总结protocol. 考前迅速做一遍，熟练
运用protocol.
UW 41 offline cases: 我当时和partner每个case仔细做一遍，虽然不像online有反馈
，但对练临床思维，补充protocol非常有帮助。最后考试与41 case有异曲同工之处，
遇到复杂case也不慌神。

最近科室里面掀起了一阵CCRN潮，科室还给大家一套AACN的复习材料给大家轮流使用。
AACN 有一盘CD 和三套模拟训练题， 我只看了CD，觉得非常有用的，每个系统把考点
总结了一下，很好的加强了理论知识。
我还用了pass CCRN CD 的题库，没有全部做完，cardio 和 pulm 实在太多了，大概各
做了70题，
其他category题目少的基本都做完了，每次无论对的错的都看解释加做笔记，虽然笔记
后来一遍都没看过 ：（
总共复习了两个星期，其中全职上班,不上班的日子每天复习8小时，上班的日子完全不
看书。
实战感觉和 pass ccrn 接近或者更简单一些，基本一分钟一题，题干也不长，磨棱两
可的做bookmark，做完全部150题 然后回头琢磨那些磨棱两可的，根本不需要三小时。
以下理论知识基本在考题中都有用到：
CVP, PAOP, PAP, CO, CI 这些正常范围一定要知道，怎么区分LVHF 和 RVHF，fluid
status,
preload, afterload,

发帖数: 688 | 8 warfarin用的比较久，医学界对它的Profile都了解的比较透彻。降低血块形成的可能
，有效降低中风的风险。当然这也意味着它的主要副作用是增加出血风险。此外，吃
warfarin的病人的饮食要很讲究（avoid leafy greens)，有一些药物之间的反应，病
人还要经常monitor inr比较麻烦。
你说你父亲已经使用warfarin2年多，inr在target内，也没有什么不适（有没有仔细
monitor bleeding signs?），应该来说是manage的比较好的病人。
Dabigatran/Apixaban/Rivaroxaban是近年来出的新药。Dabigatran是direct thrombin
inhibitor, 后两个是factor 10a inhibitor。这些药的主要特性就是不需要频繁
Monitor inr，也降低了出血风险（跟剂量有关），而且还对预防中风更有效（或者non
-inferior)。价格自然也贵。
你父亲医生想给他换apixaban，有一个trial叫ARISTOTLE,评价这个药和warfarin在

菜，目前是没有观察到bleeding signs. 所以我有些纠结有没有必要换。尤其是看到
apixaban没有antidote.......
医生当时是说可以考虑换，但也没有强烈推荐。让我们先check一下保险包不包，我自
己多想了些。呵呵

及时纠正过来。在预防中风的药效上，apix还是有优势的。 |

最近听到一首圣歌叫做谷中百合花(见国内版《新编赞美诗》第301首, 及美国华人教会
常用的《教会圣诗》第145首)， 非常喜欢其旋律。歌词中把耶稣比作谷中百合花。随
便google了一把， 得知歌词中关于谷中百合花的典故是来自旧约之雅歌2:1. 一些
传统的圣经评注认为，雅歌2：1是新郎的话，根据耶稣和教会之间的新郎-新娘的关系，推
出这是耶稣的话， 就是耶稣说"I am the rose of Sharon, the lily of the valleys
." 然而自己查看圣经后， 发现了一个问题， 并且网上也有人有同样的发现。这个问
题就是，我看见的圣经上这段话并不是新郎（lover）说的， 而是新娘(beloved)说的,
那么从新郎到耶稣的类比就无从说起了. 所以虽然这是一首在基督教 |
