牙龈红肿。然后给了拔一颗牙齿的estimate。
vet说这个治不好的，只能拔牙。我想请教一下板上的大大门，这种情况是不是必须拔
牙？我网上搜索了一下，好像也说必须拔。
另外，我还想麻烦板上有给宠物拔牙经历的亲们，帮我看看这个estimate合理不。 我
在Fremont CA，已经预约了3月9号拔牙，但如果这个收费有问题，我就立马重新多找几

啊。 生个宝贝真是九死一生， 一点都不夸张哦。
Chapter 1: 预产期前1天17号，好像她知道我终于不上班， 可以迫不及待的出来了
。 先是出血，出现宫缩， 开始进入labor stage 1 (latent phase)。好疼啊，从来没
有过痛经（考点：prostaglandin, tx NSAIDs）, 对这种痛实在不能忍受。去医院，医
学院附属，有医学生，intern, resident, attending。 告知宫开一指， ok, 回家 （

我总是混迹于小组之间。自己学的时候，学着学着，就会走神。当时没人和我一起冲刺
，我又开始磨叽了。我走神到什么程度吧，反正最后我每做一件事就得把时间记下来。
看题也是，复习每道题都是写下时间，这样自己就知道是不是又溜号了。后来幸运的是
挂上了nunu的车，被带了一阵，学会了怎么冲刺。幸亏也有doughnut做火车头。这才冲
将下来。
12月做的 uwsim1,690/254，减掉20，觉得有点玄。我这个人正式考试从来都不会让人
惊艳，万一失手，岂不230都没了。那段时间正好突发牙疼，有时候疼得简直要在地上
打滚，晚上也睡不好。看牙医吧，有次助手给打的麻药，根本没麻到神经。学艺不精害
死人啊。那一阵被折磨得不行，以至于fa小组学习时能不说话就不想开口，感觉提不上
气来。正好有理由把考试推到1月了。后来证明推迟是对的，因为12月底去考试中心模
考，3个block就觉得受不了。

俺的牙齿保卫战已经打了快3年了吧，Abscess天天有啊，没钱看牙医，没时间回国治，
估计要等到明年夏天甚至更以后了吧。
明年俺也以250为目标试一试，也是老CMG了，估计要多搞几本书看看才行了。

一个手术室最重要的就是外科医生，好比一个诊所最重要的就是医生。没有外科医生，
其他手术室的工作人员都英雄无用武之地，诊所也一样。所以其他的辅助工作人员很容
易被替换。一个诊所可以解雇所有的员工就是不能解雇医生。同样，手术室里外科医生
就是中心，其他的都是围绕着他/她转。
不管中国还是美国，麻醉都是辅助，经常就是外科医生骂麻醉医生像骂孙子一样。
而FNP是百搭。哪里都需要。精神病院也要。监狱也要。养老院也要。邮轮也要。有
refer 病人的权力。
这个人神公愤的abscess， 就给这个医生吧。听说他手最近有点抖。只能做这种手术。
我refer 的时候， 想一想。这个女病人，还是找一个帅哥外科医生可以增加幸福感。

spinal tap， 第三天night shift hospitalist告诉我MRI是normal，spinal tap不能
做， 因为病人on fragmin， 要等24hr。
我自己看了一下MRI， 这个病人有spinal abscess。 当天就手术了。
这个radiologist的误读让人很恼火， 可是这个fragmin更是火上浇油。 我不明白，
现在的hospitalist都用fragmin for DVT prophylaxis? Why not SCD + heparin for
high risk patients? 这个病人根本没有on SCD. |

Patient was treated with steriod, and symptoms get better:) We were
joked at for telling them that the imaging findings are most consistent
with CNS lymphoma, and the brain and mediastinum were separate
processes:)

very helpful reducing edema and inflammation, this is also true for
metastatic brain lesions. Regarding edema caused by bleeding/abscess, I
guess it is not very clear, it depends on each patient's clinical situation.
For CNS lymphoma, corticosteroid does have substantial anti tumor activity.
You would want to talk to hem/onc before you give steroid for somebody whom

private area and was suffering from terrible pain from the infection. His
wife was hospitalized too because of the potentially fatal fungal meningitis
2/2 steroid injection.
While asked him about his anxiety. He said that he has been anxious about
pretty much everything in his life, but he was most anxious about if he
could be discharged before the Election Day so that he could vote.

They did consider incomplete kawasaki once....but she has no other symptoms
besides fever/cough/abdominal pain. Appendix abscess is not likely based on
normal u/s although the visualization is limited. They even did HIV test
which was negative.
No PPD has done yet, and CXR does not support it apparently.
I thought mostly likely viral infection, but persisting high fever really
worrisome. Plus, she frequently wakes up due to spiking fever or bad cough

abscess and radiculopathy. 如果病人有糖尿病/高血糖病史则可能性更大。
现在就是在等血培养结果么？既然最新的MRI显示右侧腰大肌可能有积液, 或可考虑用
介入放射引导(IR-guided)对积液引流然后做细菌培养，如果医院有这条件的话。 |

呵呵 身上有无数种病的人最划算。 对绝大多数人来说是非常不划算的 小磕小碰感冒
都不cover 我这边一个病人就是obamacare， 这兄弟干活的时候被钉子戳进了拇指，
他自己给拔了出来 好像没做任何处理 结果拇指骨髓炎 感染灶扩散 腰椎骨髓炎 浑身
多处abscess 感染性休克 死亡。
劳动人民真可怜 原来想省点银子 DIY一把。结果命都搭上了。如果当时 这种例子不计
其数。
我觉得obamacare如果能学学中国的基层保险就好了 cover小病不cover大病 把钱花道
预防上 这样广大人民群众会受益。 |
