以打开，其实我chest x-ray 的结果上次给了医生，结果他没装到信封里，我想问问那
位大侠有经验，rfe 后交的I-693 还需要申请人和医生重新签名吗 ？我的 chest x-

2周前收到485RFE，当时体检时怀孕，所以没有打一些疫苗和TB test，这次RFE就让
补做TB test和填一个叫“supplemental form to i-693” 的表去解释当时为什么没打
疫苗。（因为现在还在怀孕，所以只补做了TB test，还是不能打疫苗）
现在我有一个问题，这个form上需要填写我的护照号，但我上个月刚刚换了新护照
，新护照和老护照不是一个号，当时递交485申请的时候还是用的老护照，现在这个
form上应该填新护照号还是旧护照号？如果填新护照号的话，还需要把新护照的复印件
重新再寄给移民局吗？

1. 医生选这个结果对吗？我们是不是要等疫苗都打完才交485？这样要等好久。
2. 另一个选项（3）是 "vaccine history complete for each vaccine, all
requirements met".你们体检报告是选(1)还是选(3)？

you file I-130, they file I-485, all in the same package.
All three of you must submit 出身公证.
Your father also needs his 结婚公证.
better have I-693 in your first submission, or if they don't mind waiting 2
more months in the process, wait for the RFE and then submit I-693. |

using the 10/11/11 version of Form I-693. If a medical examination

我的医生没给我打，在I-693表上check了“Not Age Appropriate”. 我在USCIS I-693
instruction里的确看见说
HHS has determined that a vaccine is "not medically appropriate" if :
a. The vaccine is not recommended for your specific age group;
可是又在USCIS上看见说Flu Season需要打Flu Shot,
"If your immigration medical examination is during this period, you are
required to have the seasonal flu vaccine."
到底需要打么？USCIS怎么说的这么模糊的啊。。 |

发帖数: 5769 | 11 好了,今天来说说填表以及资料准备,国内需要准备的资料见上一篇.
表格在USCIS网站上，把需要用的表格和instruction都印下来好好研究。
最主要的表格就是I-130和I-485
其他一些表格都是I-485附带要求的。
File I-130比较简单只需要子女的公民纸复印件，出生证复印件，父母的出生证复印件
，I-130 Fee（各个时期不一样请去USCIS网站查询）。
Mailing address也各不同，都需要去网站查询。
如果父母在美国境内，可以同时file I-485，这个资料就比较多了，如果父母签证快要
过期要早早准备，不过一旦I-485寄出，就不用申请延期，因为这样就算I-485 pending

表格在USCIS网站上，把需要用的表格和instruction都印下来好好研究。
最主要的表格就是I-130和I-485
其他一些表格都是I-485附带要求的。

但是医生非要我先去health department 作进一步评估，我告诉他我可以先拿到693再
去，他说什么来美超过五年的理论上可以先拿表再去，但是有可能485因此被拒，所以
强烈建议我先去，但需要几个星期才能预约。
我查了下，他说的skin test > 10 mm来美五年以上之类的是08年的老规矩，09年明确
改了不是非要去health department了，但他坚持说我是错的，后来我说要先拿表，他
说他会写上我来美超过5年所以不是必须要去health department，但仍然是latent TB
而且将来老了可能会得病。。。
这样我可以周一拿报告，但是他说一切责任我自负，。。。这样的693能用吗？
换医生又得麻烦，就差这一步了，所以也不想换了

（1）如果concurrent＋pp，以上所有都寄到同一个地址对吗？譬如，我在nsc，就把所
有东西一块寄给nsc？

（2）置顶文章里有说在efile后7个工作日以内supporting documents要收到。有这么
一说吗？为什么我在uscis网页上没有看到。上面只说材料到齐了才开审。如果没有这
个要求，我现在在等推荐信回来中，可以先efile吗？一来材料一到齐就可以寄，再来
如果eb1突然排期的话还可以占个早点的 PD。

发帖数: 69 | 20 由于国内注射过BCG疫苗，觉得皮试会出现假阳性。当初没有做TB皮试，直接做了X-Ray
。结果一家三个人，一个过了，另外两个被退回。附了份信说，光X-ray不够。需要皮
试结果。但医生仍坚持BCG接种者，说这样就可以。加了说明，说是BCG接种者。
但感觉，附的信说了光X-ray不够，而且查了网站说了BCG接种的人也需要做皮试。我和
医生讲，结果他很生气。说我挑战他的决定。
想请教大家对付过这种僵局吗？自己有点怕这次寄过去再被退回，比较麻烦。

本着和140一样的原则，我的485材料非常精简，比如身份材料，只提供了最后一次入境
美国之后的。我（主申请人）去年9月回国H1转H4，所以只提供了H4的签证复印件，以
前的H1和I20之类统统没提供。出生公证只复印了两页（中文和英文翻译），后面公证
页没
复印，因为觉得不相关。但加了一份声明（没公证），说本人懂中英文，可以证明英文
翻译是准确完全的。护照只复印了信息页和最后一次入境美国的签证（H4）。AP/EAD的
材料也非常精简，两个申请人，6份申请，加起来也就薄薄一点，不知道是不是因为这
样IO一高兴先拿出来批了，嘿嘿。

发帖数: 152 | 23 今天收到移民医生的电话，说血检结果我必须打破伤风的疫苗，但是必须打三针，今天
打一针，一个月后打一针，六个月以后再打一针。所以我只能先收到密封的incomplete
的693表，随后两针打好了再给我一个信封。请问有人碰到过这样的情况吗？破伤风需
要分三次打的吗？想问问大家的意见，谢谢啊！
PS：如果现在file 485的话（因为比较急等绿卡），693表incomplete会影响绿卡的
process吗，还是IO根本不会碰我的case必须等到收到所有的信息才开始处理？谢谢！ |

那么，同一个 I-140 可以提交多份 I-485 吗？
移民局说可以，并把这种 I-485 的申请称之为 Duplicate Filing. 而且，Duplicate
Filing 的 I-485 是不用交费的了。
在什么情况下，Duplicate Filing I-485 呢？比如说你的 I-485 在 NSC 搁了一段时
间了，也过了公布的 Processing Dates，于是再重交一份去 TSC，希望能快一点。
除了不用交一千多的申请费，要不要再交一份体检表 I-693？－－不知道。
同样，前面 Second Filing 的时候，体检表 I-693 是新交一份，还是可以指明在另一
份申请中已经交过了，也不清楚。
虽然有 Duplicate Filing 一说，具体是否有可操作性，因为牵扯到跨区递交的问题，
不是很确切。手头连一个这样的例子也没有。 |

我的485因为693上少了个疫苗的记录被RFE，结果比LD晚批了一个多月。在板上问了才
知道医生当时应该给个copy的。可当时自己不知道啊。提醒还没做的人注意。
提交485的时候不是flu season，所以没有打flu shot，我被RFE的时候是flu season了
，我就把flu shot也加上了。免得再次被RFE.
2.卡的邮寄问题。
我想大多数人大概都不会遇到这个问题，但是一旦遇到，真的是很头疼。我的EAD卡第
一次没有收到， 网上状态说“the post office returned your Card as
undeliverable”。打了客服电话说会再送一遍。 后来收到这个service request的确
认信， 上面说“It appears that you have failed to properly register with
postal service.” 我打电话到邮局，人家说there's no such thing as "
registering with USPS". 而且我的地址是正确的。

但是485和693上都是City/Town/Village，这个如何处理？需要和公证书保持一致，只
填写到县（市）？还是尽量填完整？
另外，693电子版，出生地一栏，也填不了那么多。需要怎么处理？
需要重新开公证书吗？

我是国内土博，J1，来美时间也不长，中间还transfer一次,没有离开过美国。
文章十几篇都是在国内发的，引用140来次，主要来自一篇挂名文章，审稿大概15次，
背景比较弱，请的律师。推荐信美3，中1，德1，没有大牛，但是都非常nice,一个字都
没有改过。律师写的PL,看上去就是模板上的套话，但是应该是都说到了重点，个人感
觉律师在140申请对我的帮助还是比较大的。
485 DIY:
结婚证是中国的，在网上找了个翻译公司，60块钱，翻译加notarize.

3月21号网上查到我被RFE，LD的approve。老大于25号收到卡。
RFE的理由是我没预料到的。其实也怪自己粗心，没有仔细看医生给的I-693 form。当
时是忍痛去体检的，月子里实在没太多精力。
Mumps virus的抗体滴度0.94。小于等于0.90算negative，0.91到1.09算可疑。医生当
时在693上写了Mumps 疫苗会在breastfeeding结束后完成。移民官认为nursing不是
contraindication。
不知道如何办是上策，虽然有84天的期限，但84天我是不会结束母乳的，也不想因为打
疫苗而停止母乳，估计停了一阵后就没了。
今天会联系体检医生，看看他的意见，毕竟我交了那么贵的体检费。
请过来人给建议。 |

因为是J换成Ｈ，中间LD用工卡工作过，所以感觉有些复杂，希望大家帮
忙检查一下。
主要有几个问题需要确认
问题1. J2用过工卡，要附上工卡的复印件就可以了吧？
问题2. 看到有些人说孩子的出生证也要加，有必要吗？
问题3. 在学校内部换工作，H1B的amendment刚提交不久，还没有批准，应该加I-129
petition表格吧？
问题4. AP那个需要离开美国的理由我是看到网上有人用了，有没有模板可以找到，是
不是不加也行啊？

whatever makes him sign your I-693 都行
你疫苗可以自己打，但是医生承认不承认，是他的自由，有承认的，也有不承认的
医生接受不接受保险也是他的自由。总之他只要签字就好。

A#，你可能没有（你OPT的EAD上的A#不是），不是I-94上的，个人认为可以空着
在485部分，你还应该提供：
i) A copy of LG's naturalization certificate
ii) A copy of LG's notarial birth certificate with English translation
iii) A copy of our marriage certificate
iv） OPT document
I-693表是有移民医生填写，你在USCIS网站上可以找一找你们城市的移民医生，打电话
问具体情况。
“问题又来了，我在一年的OPT期间就是在医院里面做了Volunteer。需不需要让我医
院提供一个证明？我在今年的4月15号之前有报过一个税表。反正是说我没有收入。像
我这个情况的话，这个税方面要怎么办呢？”

有些东西应该是不需要的，但是为了以防万一，还是准备上了。

with I-140. I made it based on others' checklists(special thanks to
orientalali).
Is anything missing or unnecessary? Should I add my I-20s since I was F-1
student before?
3 year Tax returns are necessary? Lawyer suggested, but there are too many
pages for 3 year tax returns.
